Esophageal leaks repaired by a muscle onlay approach in the presence of mediastinal sepsis.

Background: Nineteen patients were evaluated after closure of intrathoracic esophageal leaks by a pediculated muscle flap onlay repair in the presence of mediastinal and systemic sepsis.

Methods: Intrathoracic esophageal leaks with mediastinitis and systemic sepsis occurred after delayed spontaneous perforations (n = 7) or surgical and endoscopic interventions (n = 12). Six patients presented with fulminant anastomotic leaks.

Value of positron emission tomography in full-thickness chest wall resections for malignancies.

Preoperative imaging for resection of chest wall malignancies is generally performed by computed tomography (CT). We evaluated the role of (18)F-fluorodeoxyglucose (FDG) positron emission tomography (PET) in planning full-thickness chest wall resections for malignancies. We retrospectively included 18 consecutive patients operated from 2004 to 2006 at our institution.

A new neuro-FDS definition for indirect adaptive control of unknown nonlinear systems using a method of parameter hopping.

The indirect adaptive regulation of unknown nonlinear dynamical systems is considered in this paper. The method is based on a new neuro-fuzzy dynamical system (neuro-FDS) definition, which uses the concept of adaptive fuzzy systems (AFSs) operating in conjunction with high-order neural network functions (FHONNFs). Since the plant is considered unknown, we first propose its approximation by a special form of an FDS and then the fuzzy rules are approximated by appropriate HONNFs.

Collision avoidance in commercial aircraft Free Flight via neural networks and non-linear programming.

In recent years there has been a great effort to convert the existing Air Traffic Control system into a novel system known as Free Flight. Free Flight is based on the concept that increasing international airspace capacity will grant more freedom to individual pilots during the enroute flight phase, thereby giving them the opportunity to alter flight paths in real time. Under the current system, pilots must request, then receive permission from air traffic controllers to alter flight paths.

Foxp3+ T-regulatory cells in Sjogren's syndrome: correlation with the grade of the autoimmune lesion and certain adverse prognostic factors.

Sjögren's syndrome (SS) is a chronic autoimmune exocrinopathy associated with variable lymphocytic infiltration of the affected organs (primarily salivary and lacrimal glands) and broad clinical manifestations, including lymphoma development. To investigate the potential implication of Foxp3(+) T-regulatory cells in the regulation of SS inflammatory responses, we studied their incidence in the minor salivary glands (MSGs) and their relationship with histopathological and clinical disease parameters. Similar percentages of infiltrating Foxp3(+) cells were observed in the MSG lesions of all SS patients (n = 30) and non-SS sialadenitis controls (n = 7).

Video-assisted thoracoscopic resection of a small pulmonary nodule after computed tomography-guided localization with a hook-wire system. Experience in 45 consecutive patients.

Background: This study is a single-institution validation of video-assisted thoracoscopic (VATS) resection of a small solitary pulmonary nodule (SPN) previously localized by a CT-guided hook-wire system in a consecutive series of 45 patients.

Methods: The records of all patients undergoing VATS resection for SPN preoperatively localized by CT-guided a hook-wire system from January 2002 to December 2004 were assessed with respect to failure to localize the lesion by the hook-wire system, conversion thoracotomy rate, duration of operation, postoperative complications, and histology of SPN.

Results: Forty-five patients underwent 49 VATS resections, with simultaneous bilateral SPN resection performed in 4.

Robotic control in hand-assisted laparoscopic nephrectomy in humans - a pilot study.

Computer-assisted surgical devices, including laparoscopic surgical robotic arms, can be used in urologic surgery to minimize trauma. In this study, we explored the feasibility and applicability of using a robotic arm, AESOP (Computer Motion Inc, Goleta, CA), as a substitute for surgical assistants during hand-assisted laparoscopic nephrectomies in humans. Four patients (3 men and 1 woman, mean age 66 years) underwent robotic-assisted laparoscopic nephrectomy.

Kayexalate Intake (in Sorbitol) and Jejunal Diverticulitis, a Causative Role or an Innocent Bystander?

Small intestine diverticulosis is a rare entity that is asymptomatic in the majority of cases. However, it may cause serious complications, such as infection, hemorrhage, intestinal obstruction and diverticulitis. Kayexalate (sodium polystyrene sulfonate) in sorbitol has been associated with colonic necrosis and less frequently with upper gastrointestinal injuries in a subset of uremic patients treated for hyperkalemia.

Diagnosis of Birt-Hogg-Dube syndrome in a patient with spontaneous pneumothorax.

Birt-Hogg-Dube syndrome refers to a dermatologic syndrome, consisting of small papular skins lesion distributed on the scalp, forehead, face and neck, which is autosomal dominantly inherited. Subsequently patients may develop concomitant renal and thoracic pathology. We report the case of a patient with Birt-Hogg-Dube syndrome diagnosed after spontaneous pneumothorax.

Identification of cultured Brachionus rotifers based on RFLP and SSCP screening.

The marine finfish industry worldwide depends greatly on the mass culture of Brachionus rotifers. Recently, molecular data have revealed a more complicated view about the species status of Brachionus rotifers than previous mainly morphological assessments. Under this view, Brachionus rotifers are comprised of many morphologically similar, albeit genetically differentiated, cryptic members of larger groups.

Video-assisted right supradiaphragmatic thoracic duct ligation for non-traumatic recurrent chylothorax.

Background: Chylothorax is an uncommon disorder with respiratory, nutritional and immunological manifestations. Surgical management is indicated in case of recurrence or failure after conservative treatment. We report our experience with video-assisted right-sided supradiaphragmatic thoracic duct ligation for non-traumatic, non-postoperative persistent or recurrent chylothorax.

[Thoracic surgery and underfeeding scientific evidences, diagnosis and treatment].

Underfeeding causes a significant increase of postoperative complications, particularly respiratory and infectious complications. Thoracic surgery is frequently required in patients suffering wasting diseases (cancer, COPD, cystic fibrosis), which increase the risk of malnutrition. The most important risk factors are preoperative hypoalbuminemia and BMI < 20.

Herpes simplex virus type 1 in peptic ulcer disease: an inverse association with Helicobacter pylori.

Aim: To assess the frequency of herpes simplex virus type I in upper gastrointestinal tract ulcers and normal mucosa with the modern and better assays and also with a larger number of well characterized patients and controls and its relationship to Helicobacter pylori(H pylori).

Methods: Biopsy specimens from 90 patients (34 with gastric ulcer of the prepyloric area and 56 with duodenal ulcer) were evaluated. Biopsies from 50 patients with endoscopically healthy mucosa were considered as the control group.

Chemical imaging of surface reactions by multiplexed capillary electrophoresis.

A new technique for in situ imaging of surface reactions and screening heterogeneous catalysts by using multiplexed capillary electrophoresis was developed. By bundling together the inlets of a large number of capillaries, an aligned imaging probe can be created that can be used to sample directly products formed at a surface with spatial resolution determined by the outer diameter of the capillaries. In this work, we used surfaces made of platinum, iron, or gold wires to generate electrochemical products for imaging.

[Hemilithotomy position for intramedullary nailing of the femur and compartment syndrome of the healthy leg].

The well leg compartment Syndrome following long procedures in the dorsal lithotomy or hemi-lithotomy position is a rare complication. Its diagnosis is usually late and the neurological deficit are often permanent. We report two cases of femoral nailing complicated by Compartment Syndrome of the contralateral leg.

Adenocarcinoma in the anal canal after ileal pouch-anal anastomosis for ulcerative colitis using a double stapling technique: report of a case.

A case of adenocarcinoma, developed in the anal canal after ileal pouch-anal anastomosis for ulcerative colitis using a double stapling technique, is reported. In this case a T3N0 cecal cancer was found unexpectedly in the colectomy specimen. Two years later, this patient presented with an outlet obstruction of the pouch because of development of an adenocarcinoma of the anal canal.

Concurrent conventionally factionated radiotherapy and weekly docetaxel in the treatment of stage IIIb non-small-cell lung carcinoma.

Docetaxel has shown remarkable radiosensitizing in vitro properties. In a previous phase I/II dose escalation study in non-small-cell lung cancer (NSCLC) we observed a high response rate after concomitant boost radiotherapy and weekly docetaxel. The maximum tolerated dose was 30 mg m(-2) week(-1).

Dynamical Neural Networks that Ensure Exponential Identification Error Convergence.

Classical adaptive and robust adaptive schemes, are unable to ensure convergence of the identification error to zero, in the case of modeling errors. Therefore, the usage of such schemes to "black-box" identification of nonlinear systems ensures-in the best case-bounded identification error. In this paper, new learning (adaptive) laws are proposed which when applied to recurrent high order neural networks (RHONN) ensure that the identification error converges to zero exponentially fast, and even more, in the case where the identification error is initially zero, it remains equal to zero during the whole identification process.

Neural adaptive regulation of unknown nonlinear dynamical systems.

With this paper we extend our previous work on the subject, by including the case where the number of control inputs is different from the number of states which is frequently faced in control engineering problems. Uniform ultimate boundedness of the state and uniform boundedness of all other signals in the closed loop is guaranteed. Robustness of our algorithm due to the presence of a modeling error term which has linear growth with unknown growth coefficient is also established.

Convergence properties of a class of learning vector quantization algorithms.

A mathematical analysis of a class of learning vector quantization (LVQ) algorithms is presented. Using an appropriate time-coordinate transformation, we show that the LVQ algorithms under consideration can be transformed into linear time-varying stochastic difference equations. Using this fact, we apply stochastic Lyapunov stability arguments, and we prove that the LVQ algorithms under consideration do indeed converge, provided that some appropriate conditions hold.

Matrix metalloproteinases and processing of pro-TNF-alpha.

Tumor necrosis factor-alpha (TNF-alpha) is released from a cell membrane-anchored precursor by proteolytic cleavage. We have shown that broad spectrum synthetic inhibitors of matrix metalloproteinases (MMPs) prevent the processing of the TNF precursor but do not inhibit the release of other cytokines. Purified MMPs, stromelysin, matrilysin, collagenase, and the gelatinases can all cleave a recombinant pro-TNF substrate to yield mature TNF.
