We report the case of a 15-year-old boy with severe inflammatory blepharitis who was being treated with a TNFα inhibitor for Crohn's disease (CD). This case shows the potential utility of a skin biopsy in distinguishing metastatic CD associated with granulomatous blepharitis from a paradoxical blepharitis induced by anti-TNFα. It also demonstrates that ustekinumab can be considered in patients with CD who cannot tolerate TNFα inhibitors because of cutaneous paradoxical reactions.

Aims: Azathioprine (AZA) and 6-mercaptopurine are prescribed in acute lymphoblastic leukaemia (ALL) and inflammatory bowel diseases (IBD). Metabolism to active 6-thioguanine (6TGN) and 6-methylmercaptopurine nucleotides (6MMPN) is variable but therapeutic drug monitoring (TDM) remains debatable. This study reports on factors impacting on red blood cell (RBC) metabolites concentrations in children to facilitate TDM interpretation.

Article Synopsis
  • Ulcerative proctitis (UP) is a rare form of ulcerative colitis in children, with the study analyzing clinical features and outcomes of 196 diagnosed patients aged under 18 from 2016 to 2020.
  • Most patients presented with symptoms like bloody stools and abdominal pain, and the study found that higher initial disease activity scores (PUCAI) were linked to worse outcomes, including the need for more aggressive treatments.
  • By the end of the study, nearly half of the patients showed disease progression, and only a small percentage required colectomy, highlighting the challenges in managing UP in pediatric patients.

Thiopurine drugs azathioprine (AZA) and 6-mercaptopurine (6-MP) are used extensively in pediatric and adult patients with inflammatory and neoplastic diseases. They are metabolized to 6-thioguanine nucleotides (6-TGN) or to 6-methyl-mercaptopurine nucleotides (6-MMPN). The balance between 6-TGN and 6-MMPN is highly variable and monitoring is recommended, but its benefit in outcome gives rise to conflicting results, potentially increased by differences in quantifying 6-MP metabolism.

Article Synopsis
  • The study investigates the rare association between IgA vasculitis (IgAV) and inflammatory bowel disease (IBD), primarily in patients treated with anti-TNF-α therapy.
  • Out of 43 cases analyzed, the majority showed that IBD, particularly Crohn's disease, preceded the onset of IgAV by an average of 9.2 years, with many patients experiencing purpura and joint issues.
  • The findings suggest that discontinuing anti-TNF-α can lead to remission of IgAV, but also raises the risk of IBD flare-ups, while continuing treatment may help manage IBD but lead to recurring IgAV symptoms.

Hidradenitis suppurativa (HS) is a rare, debilitating skin disease characterized by the presence of recurrent tender subcutaneous nodules that develop into abscesses and fistulae. Isolated perineal Crohn's disease (CD) is unusual, diagnosis can be difficult, and distinction from HS is a challenge for the gastroenterologist. The aim of this work was to determine the criteria that distinguish perineal CD from perineal HS.

Objective: The aim of our study was to examine longitudinal changes in bone mineral density (BMD) of children and adolescents with Crohn disease (CD), and risk factors related to low BMD.

Patients And Methods: All patients ages from 2 to 18 years with CD who underwent dual-energy X-ray absorptiometry (DXA) at diagnosis and at the end of follow-up between 1999 and 2018 were considered for inclusion in this retrospective study. Factors related to changes in BMD at diagnosis and during follow-up were investigated.

Article Synopsis
  • This study evaluates the reliability of three different ELISA assays for measuring infliximab trough levels (IFX-TL) and antibodies against infliximab (ATI) in children with inflammatory bowel diseases (IBD).
  • The findings show that the three assays provide comparable IFX-TL results with a strong statistical correlation, though there are some discrepancies in therapeutic interval classifications, especially with one specific assay.
  • Overall, while the assays agree well, caution is advised in interpreting results that are close to therapeutic thresholds due to potential variations in classifications.

Article Synopsis
  • TNF-α inhibitors, commonly used for chronic inflammatory diseases, can sometimes trigger unexpected immune-mediated inflammatory reactions, known as paradoxical effects.
  • This study presents three cases of paradoxical chronic recurrent multifocal osteomyelitis in patients being treated with infliximab or adalimumab for Crohn disease, all of whom experienced bone pain without joint issues.
  • Despite being in remission from their Crohn disease, patients developed these symptoms after 2 to 26 months of anti-TNF therapy, but all recovered after stopping the medication, highlighting the need for clinicians to recognize such rare side effects.

Infliximab (IFX) is administered intravenously using weight-based dosing (5 mg/kg) in inflammatory bowel disease (IBD) patients. Our hypothesis is that especially young children need a more intensive treatment regimen than the current weight-based dose administration. We aimed to assess IFX pharmacokinetics (PK), based on existing therapeutic drug monitoring (TDM) data in IBD patients < 10 years.

Background And Aims: Nucleotide oligomerization domain 2 [NOD2] mutations are key risk factors for Crohn's disease [CD]. NOD2 contributes to intestinal homeostasis by regulating innate and adaptive immunity together with intestinal epithelial function. However, the exact roles of NOD2 in CD and other NOD2-associated disorders remain poorly known.

Objective: Infliximab (IFX) is a frequent therapeutic option for Crohn disease (CD) patients. Early detection of responders to IFX is critical for the management of CD in order to avoid long-term exposure to the drug without benefit. This retrospective study aimed at analysing which early parameters recorded during the induction period are able to predict response to IFX during the maintenance period in pediatric CD.
