The incidence of thromboembolism in patients with cancer is approximately 11%, and the risk of thrombosis in patients with malignant tumors is 6-fold higher than that in healthy persons. Thrombosis not only disrupts the treatment of cancer but also induces deterioration of quality of life (QOL). Knowledge about thrombus treatment is limited, and evidence is scarce.

It was reported that pulmonary embolism rarely occurs in Japan, and physicians did not think about the prevention of venous thromboembolism until about 20 years ago. Recently, however, it has been reported that pulmonary embolisms are on the increase in Japan. We previously prepared TNP-470 containing microspheres composed of poly (lactic acid) with medium-chain triglyceride, and demonstrated that the microspheres released TNP-470 over the long-term in vitro. The purpose of this study was to determine the safety and efficacy of open intraoperative radiofrequency ablation of unresectable hepatic malignancies with size larger than 4 cm in diameter and/or more than three in number.
Methodology: Between May 2000 and September 2003, 30 patients (24 men, 6 women; age range, 59-72 years; mean age, 63 years) with 51 hepatic malignancies.
How much should be done for better managements of the perioperative thromboprophylaxis? In Japan, the guideline for prevention of venous thromboembolism (VTE) was established for the first time in February, 2004. Our academic medical center formed a committee for prevention of thrombosis for all departments using operating rooms in October, 2002 and began to make the database of symptomatic pulmonary thromboembolism (PTE). From the end of September, 2003, we asked the surgeons to indicate the PTE risk grades when ordering surgical operations.

Methods: Twenty-five patients who were histologically proven to have locally advanced pancreatic cancer or pancreatic cancer with distant metastases were initially enrolled in the present study. They were treated with gemcitabine infusion at either a dose of 1000 mg/m2 over 30 min (the standard regimen) on days 1, 8, and 15 of every 4-week cycle or at a dose of 250 mg/m2 over 30 min every week.

Background/aims: Anastomotic leak at the pancreatojejunostomy remains a common and dreaded complication after pancreaticoduodenectomy. Our aim was to determine the incidence, presentation, methods of management, and preoperative and postoperative factors that influence the clinical outcome.
Methodology: We reviewed our collective experience with 70 consecutive patients from 1985 to 2000 for a variety of pathologic indications.
TNP-470 (AGM-1470, 6-0-(N-chloroacetylcarbamoyl)-fumagillol), a derivative of fumagillin, is a promising angiogenesis inhibitor. However, as TNP-470 is very unstable in in vitro and in vivo, it has been difficult to verify its pharmacological efficacy in the clinical medicine. The preparation of a drug delivery system (DDS) in a microsphere form was studied for the stable inclusion and controlled release of TNP-470.
