[Efficacy of surgical treatment of medullary thyroid carcinoma in patients with regional and distant metastases].

Results of surgical treatment of 93 primary patients with medullary thyroid carcinoma within 1995-2009 years, including 26 (28%) - with hereditary disease (MEN2 syndrome) were analyzed. The best long-term results were observed in group of noninvasive tumor without metastases at the time of operation (39% - clinical and biochemical recovery, 32% - clinical remission). No one patient with extrathyroid tumor invasion and regional/distant metastases has completely recovered.

[Dynamics of morbidity of the thyroid gland cancer. Evolution of views on its diagnosis and treatment (according to material of the clinic for 50 years)].

For 50 years 603 patients with the thyroid gland cancer were operated on, to whom 715 operative interventions were done. Before the Chernobyl disaster 395 patients were examined, after her--208. Differentiated forms of the thyroid gland cancer were diagnosed in 97.

[Pathogenesis, surgical treatment and prophylaxis of goiter recurrence].

The causes of the nontoxic goiter recurrency occurrence, the prophylactic methods and the results were analysed according to the data obtained while examination and treatment of 115 patients. The autoimmune damage of a leaved thyroidal gland remnant constitutes one of the causes of the genuine recurrency of goiter.

[The immune status indices of patients with local purulent necrotic lesions].

Hypoproteinemia, the lowering of albumin-globulin coefficient, IgG and T-suppressors level, the circulating immune complexes contents increase were revealed while the immunological state investigation in 100 patients with purulent wound. The results obtained witness the necessity of application of immunostimulating and antimicrobic methods, which do not influence an organism negatively (fluorochinolones, for example) in treatment of patients with purulent-necrotic infection.

[The diagnosis and surgical treatment of a lateral cyst and fistula of the neck].

Surgical treatment was performed in 66 patients with lateral cyst and 23 with congenital single brachiogenic cervical fistula. In 14 patients, a cyst was complicated by a cervical phlegmon, 17 had a complete fistula, 6--incomplete one. In cyst diagnosis, the clinical signs and puncture of a cyst is of crucial importance, in fistula diagnosis--its connection with the pharynx--introduction of a dye.

[Preoperative preparation and surgical treatment of elderly and very old patients with abdominal hernia].

On the basis of analysis of the results of treatment of 312 elderly and senile patients with abdominal hernia, it was established that performance of adequate preoperative preparation permitted considerable extension of indications for the operation. Contraindications for surgical treatment of such patients should be established after performance of preoperative preparation with regard for indices of external respiration and hemodynamics.

[The surgical treatment of toxic goiter and the prevention of its complications in light of current concepts of the pathogenesis of the diseases].

The authors have established in patients with toxic goiter the rough disorders in immune status retaining after surgical treatment. Immunocorrection on the background of surgical treatment is pathogenetically substantiated and contributes to decrease in incidence of the development of postoperative hypothyrosis and disease recurrence.

[Clinico-functional evaluation of the effectiveness of preoperative care of patients with postoperative abdominal hernia].

In 27 patients with postoperative abdominal hernias, on the basis of studying the indices of respiration, hemodynamics, ABB and blood gas, it was established that preoperative preparation lasting 9-12 days provided adaptation of the respiratory and circulatory systems to the increased intraabdominal pressure.

[The so-called goiter of the accessory thyroid glands].

In the period of 1954-1966, 74 patients were operated on for goiter of the accessory thyroid glands (GATG). Of them, in 45 in histologic investigation, papillary adenoma or microfollicular, less often parenchymatous, goiter were revealed, in 29--papillary cancer or papillary adenoma at the state of malignization. The long-term follow-up (up to 30 years) of the patients with reoperations for GATG recurrences, or subsequent development of a "goiter" has shown that GATG presented a metastasis of papillary thyroid cancer into the lateral lymph nodes of a neck.

[Characteristics and surgical treatment of nodular goiter in middle-aged and aged patients].

Four hundred and two elderly and senile patients with nodular goiter were operated on. In most of them, hypothyrosis (decreased triiodothyronine level and increased thyrotropic hormone content in the blood) was revealed; in all--concomitant diseases (2.9 per a patient) and immunologic disorders; in 47%--cervico-retrosternal location of a goiter.

[Surgical treatment of cholecystitis and cholecystopancreatitis].

The treatment of 3539 patients has shown that patients with the unquestionable diagnosis of acute cholecystitis (cholecystopancreatitis) without aggravating circumstances must be operated upon in earlier time - within 3--6 days from the onset of the disease, in spite of the improved general condition. Other conditions being equal, the question of surgical treatment must be solved earlier in elderly and senile patients with acute cholecystitis and cholecystopancreatitis.
