Background: Trans-sacral axial L5/S1 interbody fusion (AxiaLIF), a novel surgical procedure, recently adopted in clinical practice, has excellent clinical outcomes. However, there is inadequate data on the feasibility of the approach in all adult patients and the optimal surgical approach is currently unclear; therefore, further studies are required. In order to enhance the surgical approach for AxiaLIF, prospective anatomical imaging optimization is necessary.

Article Synopsis
  • Previous research on axial lumbar interbody fusion (AxiaLIF) has primarily focused on the L5/S1 segment, with no prior data on its feasibility for both L4/5 and L5/S1 fusions.
  • This study involved 40 subjects who underwent MRI scans to analyze spinal measurements and assess the safety of the transsacral axial interbody fusion approach for both intervertebral levels.
  • Results indicated that only 20 subjects had a viable reference line for fusing both levels, suggesting that while the approach is feasible for some, it poses challenges for others.
