However, its potential is restrained by unique supply chain challenges, including dynamic patient health conditions and extended turnaround time. These challenges often lead to missed optimal treatment windows, impeding the effective delivery of life-saving treatments.

Implementing Tourniquet Conversion Guidelines for Civilian EMS and Prehospital Organizations : A Case Report and Review.

Since the first documented use of a tourniquet in 1674, the popularity of tourniquets has waxed and waned. During recent wars and more recently in Emergency Medical Services systems, the tourniquet has been proven to be a valuable tool in the treatment of life-threatening hemorrhage. However, tourniquet use is not without risk, and several studies have demonstrated adverse events and morbidity associated with tourniquet use in the prehospital setting, particularly when left in place for more than 2 h.

Fecal microbiome transplantation and tributyrin improves early cardiac dysfunction and modifies the BCAA metabolic pathway in a diet induced pre-HFpEF mouse model.

More than 50% of patients with heart failure present with heart failure with preserved ejection fraction (HFpEF), and 80% of them are overweight or obese. In this study we developed an obesity associated pre-HFpEF mouse model and showed an improvement in both systolic and diastolic early dysfunction following fecal microbiome transplant (FMT). Our study suggests that the gut microbiome-derived short-chain fatty acid butyrate plays a significant role in this improvement.

Paleogene microsyopid plesiadapiforms are among the oldest euarchontans known from relatively complete crania. While cranial endocasts are known for larger-bodied Eocene microsyopine microsyopids, this study documents the first virtual endocast for the more diminutive uintasoricine microsyopids, derived from a specimen of Niptomomys cf. Niptomomys doreenae (USNM 530198) from the late Paleocene of Wyoming.

Capacity planning in a decentralized autologous cell therapy manufacturing network for low-cost resilience.

The goals for increased patient access and fast fulfillment have motivated considerable interest in autologous cell therapy manufacturing networks having multiple and geographically distributed manufacturing facilities. However, the cost of safety manufacturing capacity to mitigate supplier disruption risk-a significant risk in the emerging cell manufacturing industry-would be lower if manufacturing is centralized. In this paper, we analyze a decentralized network that has as its objective to minimize the cost of network resilience for mitigating supplier disruption by making use of the fact that bioreactors for autologous therapy manufacturing are small enough to be relocatable.

Anaphylaxis is a life-threatening condition with a known effective prehospital intervention: parenteral epinephrine. The National Association of EMS Physicians (NAEMSP) advocates for emergency medical services (EMS) providers to be allowed to carry and administer epinephrine. Some states constrain epinephrine administration by basic life support (BLS) providers to administration using epinephrine auto-injectors (EAIs), but the cost and supply of EAIs limits the ability of some EMS agencies to provide epinephrine for anaphylaxis.

Prior work demonstrated that a splice variant of SCN5A, a voltage-gated sodium channel gene, acts as a cytoplasmic sensor for viral dsRNA in human macrophages. Expression of this channel also polarizes macrophages to an anti-inflammatory phenotype in vitro and in vivo. Here we utilized global expression analysis of splice variants to identify novel channel-dependent signaling mechanisms.

The house/ball community is an understudied sub-group of young Black men who have sex with men and transgender persons in urban centers of the United States who affiliate in social structures called houses and gather at elaborate dance and performance events called balls. In Charlotte, North Carolina, 12 house/ball members were interviewed about their experiences with health care providers and their assessment of any barriers to care due to their affiliation with the rather clandestine house/ball sub-culture. Additionally, HIV-specific health care providers were interviewed, to assess their knowledge of the sub-culture.

Field spinal immobilization using a backboard and cervical collar has been standard practice for patients with suspected spine injury since the 1960s. The backboard has been a component of field spinal immobilization despite lack of efficacy evidence. While the backboard is a useful spinal protection tool during extrication, use of backboards is not without risk, as they have been shown to cause respiratory compromise, pain, and pressure sores.
