A cohort of 28 VLBW (less than or equal to 1500 g) infants was assessed at 1 and 3 years of age for hearing, language development and neurological status. Language delays were detected in 11 (39%) infants at 1 year, and in four (15%) at follow up at 3 years of age (P less than 0.05).

Background: While regulations for workplace lead exposure become more strict, their effectiveness in decreasing blood lead concentrations and the method by which this is attained have not been evaluated.

Methods: An analysis was conducted of 10,190 blood lead samples from employees of 10 high-risk workplaces collected in Manitoba, 1979-87, as part of regulated occupational surveillance.

Results: A significant decrease in blood lead concentrations was observed overall as well as for each individual company.

Objective: To determine the optimal management of catheter-acquired bacteriuria after short-term catheter use in women.

Patients: Asymptomatic patients (119) with catheter-acquired bacteriuria were randomly assigned to receive no therapy, a single dose (320-1600 mg) of therapy with trimethoprim-sulfamethoxazole, or 10 days (160-800 mg twice daily) of therapy. Thirty-two patients with lower tract symptoms alone received a single dose or 10 days of therapy, and 10 patients with upper tract symptoms or signs received 10 days of therapy.

Salazopyrine, a common therapy for inflammatory bowel disease, is known to have a reversible antifertility effect in men and male rats via its metabolite, sulphapyridine. To determine if there is an adverse effect in females, 20 mature Sprague-Dawley rats were assigned to a treatment (400 mg/kg sulphapyridine daily), or a control group. Twenty obligate carriers of infantile hypophosphatasia (HOPS), a severe autosomal recessive metabolic bone disorder, were studied and compared with 36 controls. Decreased serum alkaline phosphatase activity and increased urinary phosphoethanolamine excretion were confirmed in the HOPS carriers. Diminished responsibility is based on the notion that mental disorder may make a person only partially accountable for his act in the killing of another person. For this plea to succeed it has to be shown that an accused has to be suffering from such an abnormality of mind that his responsibility for the action is substantially diminished. In order to provide an overview of the relative contribution of platelet, von Willebrand factor, and other abnormalities to patients with clinical bleeding difficulties, we performed a retrospective survey of coagulation studies on 569 individuals referred to the University of Manitoba coagulation laboratory because they, or a closely related family member, showed clinical evidence of a bleeding disorder. There was a highly significant (p less than 0.001) negative correlation between the bleeding time and each of the following parameters: the platelet count; the hematocrit; the percent aggregation to collagen, epinephrine, ADP, and arachidonic acid; and the logarithm of von Willebrand factor antigen and a measure of its activity (ristocetin cofactor).

In a survey of chronic respiratory symptoms and airflow obstruction of a rural population aged 20-65 years, the effects of both farming and, in particular, the exposure to grain dust on the respiratory health of the people was examined. Of the 1,892 subjects (82% of the eligible population) studied, 924 were males and 968 females, with half living in the town. Three groups were identified: current, former, and never farmers.

We have prospectively reviewed the data on 52 children who presented with status epilepticus. Thirty-four (65%) of the 52 had not had seizures before. Children who were previously abnormal were more likely to present with partial status epilepticus or to have seizures greater than 60 minutes than those who were previously normal.

The relationships between transient cerebral and retinal ischemic symptoms and brachiocephalic angiographic findings were studied in 464 patients. A 6-year follow-up study of height and weight was conducted between 1975 and 1983 in a total of 149 sibling pairs of the same sex, one of whom had been exposed to diagnostic ultrasound in utero. The data were analyzed for the entire sample group, as well as for subgroups defined by sex, gestational age at delivery, birth order, and gestational age at time of first exposure to ultrasound. No statistically significant differences of head circumference at birth or of height and weight between birth and 6 years of age were found between ultrasound-exposed and unexposed siblings.

To evaluate the effect of 1-desamino-8-D-arginine vasopressin (desmopressin) on blood loss in surgery, we conducted a randomized, double-blind trial of the drug in 35 patients with normal hemostatic function who were having spinal fusion with Harrington rod instrumentation. Seventeen patients were designated to receive 10 micrograms/m2 of desmopressin, and 18, to receive a placebo. Preoperative testing showed that desmopressin increased factor VIII coagulant activity, von Willebrand antigen concentrations, glass bead platelet retention, and prothrombin consumption and decreased the partial thromboplastin and bleeding times (p less than or equal to 0.

History of acute symptoms (cough, wheezing, shortness of breath, fever, stuffy nose, and skin itching/rash) following exposure to grain dust was obtained from 661 male and 535 female current and former farmers. These symptoms were relatively common: 60% of male and 25% of female farmers reported at least one such symptom on exposure to grain dust. Association of cough, wheezing, shortness of breath, and stuffy nose with skin reactivity and capacity to form IgE is consistent with an allergic nature of these symptoms.
