Calciphylaxis is a rare condition characterised by painful necroses due to microvascular calcifications. It primarily affects individuals with end-stage renal disease and affected calcium-phosphate metabolism. This is a case report of a single-stage secondary breast reconstruction after mastectomy using the excided tissue from the contralateral reduced breast based on the perforant vessels of the internal thoracic artery. We used this technique in a 62-year-old woman with a hypertrophic contralateral breast, who had undergone mastectomy due to breast cancer.

Further investigations revealed, that she had previously undergone an autologous lipografting from her thighs to her gluteal area, and she had developed widespread, severe hygroma formation as a late complication to the procedure. The complication rate to liposuction and -grafting varies 7-10% with seroma being the most abundant.
