Takotsubo cardiomyopathy manifests as global myocardial hypokinesis, a rare challenge for anesthesia practitioners. This report discusses a case in which a seemingly healthy patient presented for open abdominal hysterectomy and experienced intraoperative cardiac arrest requiring cardiopulmonary resuscitation. Takotsubo cardiomyopathy was diagnosed following resuscitation.
