Background/aims: Elevated intra-abdominal pressure (IAP) can lead to intra-abdominal hypertension (IAH) and, in severe cases, abdominal compartment syndrome (ACS) in patients with cirrhosis and ascites. Paracentesis reduces IAP and improves abdominal perfusion. Intra-abdominal hypertension can also trigger acute-on-chronic liver failure (ACLF) in decompensated cirrhosis.

Article Synopsis
  • - The study aimed to predict stages of liver fibrosis and necroinflammation using various ultrasound measurements, specifically 2D shear wave elastography (2D-SWE), transient elastography (Fibroscan), and shear wave dispersion (SWD).
  • - Significant findings showed that 2D-SWE and TE effectively detected different stages of hepatic fibrosis, with specific cutoff values leading to reliable categorizations, while SWD was useful for identifying levels of necroinflammation.
  • - The conclusion emphasizes that both 2D-SWE and TE provide a noninvasive means to assess liver damage stages, with SWD being beneficial for detecting inflammation in the liver.

  • Laparoscopic sleeve gastrectomy (LSG) is a common weight loss surgery that may affect gastroesophageal reflux disease (GERD), but research using objective tests like multichannel intraluminal impedance combined with pH testing (MII-pH) is limited.
  • A study involving 33 patients undergoing LSG found significant increases in acid reflux and pathologic reflux after the procedure, indicating a potential worsening of GERD symptoms post-surgery.
  • The study suggests that preoperative and postoperative evaluations using endoscopy and MII-pH testing are important for monitoring GERD, highlighting the need for closer follow-up after LSG.

Crohn's disease is an inflammatory, autoimmune disorder that predominantly affects the intestines but can also affect extraintestinal organs. Certain neurological conditions, such as autoimmune encephalitis, can develop along with this disease. In this case report, we present a case of anti-glutamic acid decarboxylase (GAD) antibody-associated autoimmune encephalitis that occurred shortly after the diagnosis of Crohn's disease and was unrelated to the treatment and nutritional deficiencies.

Article Synopsis
  • - Chronic hepatitis B virus (HBV) infection can lead to hepatocellular carcinoma (HCC), and this study explores the methylation status of tumor suppressor genes RASSF1 and CDKN2AIP in patients with HBV-induced liver cirrhosis.
  • - The study involved 47 patients divided into two groups: those with both HCC and liver cirrhosis (22 patients), and those with only liver cirrhosis (25 patients).
  • - Results indicated that methylation levels of the CDKN2AIP gene were significantly lower in patients with HCC, suggesting that cfDNA methylation of these genes could be valuable for diagnosing HCC in HBV cirrhosis patients

Objective: Esophageal motility is regulated both by coordinated stimulation and inhibition of the circular and longitudinal muscle layers of the esophagus. Although there are many diseases known to have an effect on esophageal motility, the effect of subepithelial lesions (SELs) of the esophagus on esophageal motility, which is often detected incidentally, remains still unclear. The aim of this study is to reveal the effect of SELs of the esophagus on esophageal motility evaluating it by high-resolution manometry (HRM).

Article Synopsis
  • Occult hepatitis B infection (OBI) occurs in individuals who test negative for hepatitis B surface antigen (HBsAg) but still have the virus's genome in their liver, indicated by the hepatitis B core antibody (anti-HBc).
  • A study of 59 HBsAg-negative chronic hepatitis C patients found OBI in 27.1% of cases, particularly in those who were anti-HBc positive, while none had detectable serum HBV DNA.
  • The study suggests that anti-HBc positivity is linked to liver injury progression and recommends screening for anti-HBc in chronic hepatitis C patients to monitor potential complications.
