Infective endocarditis is an infectious disease of the heart tissue, mainly affecting heart valves and intracardiac devices. We present the case of a 71-year-old male pacemaker carrier with a history of hepatic cirrhosis, esophageal varices, hepatocellular carcinoma, and severe aortic stenosis, who was admitted to the emergency room and hospitalized due to upper gastrointestinal bleeding. Although upper endoscopy showed no signs of active acute hemorrhage, the patient required a red blood cell transfusion.

Brucellosis is a commonly diagnosed zoonosis and neurological involvement is rare. A 30-year-old woman presented with a pulsatile headache that was exacerbated by the Valsalva maneuver and refractory to analgesic therapy. The patient also had nausea, cough, and coryza that evolved over 7 days.

Introduction: Severe sepsis and septic shock are common conditions with high levels of morbi-mortality surpassing those of coronary heart disease or stroke. The reality of hospital treated sepsis is largely unknown outside of the intensive care unit.
