: Individuals with severe mental illness live, on average, up to 30 years less than the general population, with cardiovascular disease being the leading cause of death. Metabolic syndrome (MetS) plays a significant role in this, making it crucial to manage this issue in individuals with psychosis at the onset of the illness. The approach to managing this issue has evolved from a focus on calorie counting to a deeper understanding of hormone function, particularly the role of insulin resistance in MetS.

The application of personalized medicine in patients with first-episode psychosis (FEP) requires tools for classifying patients according to their response to treatment, considering both treatment efficacy and toxicity. However, several limitations have hindered its translation into clinical practice. Here, we describe the rationale, aims and methodology of (the FarmaPRED-PEP project), which aims to develop and validate predictive algorithms to classify FEP patients according to their response to antipsychotics, thereby allowing the most appropriate treatment strategy to be selected.

Background: Estimating the risk of developing bipolar disorder (BD) in children and adolescents (C&A) with depressive disorders is important to optimize prevention and early intervention efforts. We aimed to quantitatively examine the risk of developing BD from depressive disorders and identify factors which moderate this development.

Methods: In this systematic review and meta-analysis (PROSPERO:CRD42023431301), PubMed and Web-of-Science databases were searched for longitudinal studies reporting the percentage of C&A with ICD/DSM-defined depressive disorders who developed BD during follow-up.

Background: Despite evidence suggesting that age moderates the response to preventative treatment for those at clinical high risk of psychosis (CHR-P), no meta-analysis has assessed the effectiveness of preventative interventions for CHR-P children and adolescents. Our aim was to synthesise evidence assessing preventative interventions on a wide range of mental health outcomes for CHR-P children and adolescents.

Method: A systematic search was conducted on Ovid MEDLINE, Pubmed, APA PsycInfo and Web of Science until June 2024 (PROSPERO: CRD42023406696).

Background: The Edwards Sapien percutaneous valve (Edwards Lifesciences, Irvine, CA, USA) is a promising therapeutic option for congenital atrioventricular disease mostly because of the possibility to accommodate somatic growth with balloon dilatation.

Case Summary: This article reports the performance of the Edwards Sapien 3 valve in atrioventricular valve position in four paediatric patients.

Discussion: Despite aggressive antiplatelet and anticoagulation strategies, most patients showed early bioprosthesis dysfunction, with increasing gradient not related with somatic growth.

Article Synopsis
  • Patients with head and neck cancer (HNC) often experience significant mental health challenges, particularly depressive and anxiety symptoms, especially during and after radiotherapy (RT).
  • A systematic review of 18 studies with nearly 2,000 participants revealed a peak in depressive symptoms shortly after RT, with rates decreasing over time, while anxiety symptoms showed a continuous decline.
  • Demographics indicated that female and married patients had higher levels of depression, while those who had surgery reported fewer anxiety symptoms; highlighting the need for mental health support during the RT process.

Article Synopsis
  • Genomic data has enhanced phylogenomic studies by aiding in divergence time estimation, but it presents challenges like gene tree discordance, estimation errors, and computational limits in applying complex models.
  • Using a firefly dataset with 436 loci from 88 species, researchers investigated these challenges, particularly focusing on model violations and their impact on gene tree estimation.
  • Results indicated that gene tree discordance stemmed mainly from estimation errors rather than biological factors, and despite these errors, divergence time estimations remained robust if based on well-chosen data subsets with reliable topologies.

Article Synopsis
  • Neonatal hypoxia-ischemia is a leading cause of infant mortality and disability, with therapeutic hypothermia being the only proven treatment, yet it's not always effective for severely affected infants.
  • Researchers tested the antioxidant dimethyl fumarate (DMF) on neonatal rats using a hypoxia-ischemia model, comparing it to untreated groups and sham controls.
  • The results showed that DMF significantly reduced brain damage, improved neuropathological scores, and decreased inflammation markers, suggesting it could be a potential therapeutic option for neonatal encephalopathy, though further studies are necessary before considering it for human infants.

Patients with schizophrenia receiving antipsychotic treatment present lower mortality rates than those who do not. However, the non-adherence rate is high, which can be partially addressed using long-acting injectable (LAI) antipsychotics. The impact of LAI treatments on all-cause mortality compared to oral antipsychotics remains unclear.

Sexual dimorphism arises because of divergent fitness optima between the sexes. Phenotypic divergence between sexes can range from mild to extreme. Fireflies, bioluminescent beetles, present various degrees of sexual dimorphism, with species showing very mild sexual dimorphism to species presenting female-specific neoteny, posing a unique framework to investigate the evolution of sexually dimorphic traits across species.

Background: The Molecular International Prognostic Scoring System (IPSS-M) has improved the prediction of clinical outcomes for myelodysplastic syndromes (MDS). The Artificial Intelligence Prognostic Scoring System for MDS (AIPSS-MDS), based on classical clinical parameters, has outperformed the IPSS, revised version (IPSS-R). For the first time, we validated the IPSS-M and other molecular prognostic models and compared them with the established IPSS-R and AIPSS-MDS models using data from South American patients.

Objetive: Percutaneous occlusion of patent ductus arteriosus (PDA) has classically been performed entirely by fluoroscopy, however in recent years, transthoracic echocardiography (TE) has been used as an aid to fluoroscopy or entirely by echocardiography, which avoids access of femoral artery, use of contrast and decrease in time and dose of radiation exposure. The objective of this study was to evaluate the success rate with the use of TE in percutaneous PDA closure.

Material And Method: Descriptive, comparative, retrospective study between patients in whom PDA closure was performed with fluoroscopy plus angiography (group 1) and fluoroscopy plus ET (group 2), between January 2018 and December 2022.

Despite the functional impact of cognitive deficit in people with psychosis, objective cognitive assessment is not typically part of routine clinical care. This is partly due to the length of traditional assessments and the need for a highly trained administrator. Brief, automated computerised assessments could help to address this issue.
