Especially in idiopathic thoracolumbar scoliosis the approach and sort of instrumentation have controversially been discussed over the past years. Since 1986 32 patients with adolescent thoracolumbar scoliosis underwent operative treatment, 20 patients were treated with Cotrel-Dubousset Instrumentation (CDI), 10 patients underwent Ventral Derotation Spondylodesis (VDS). The advantages of VDS according to Zielke with excellent 3-dimensional correction and shorter fusion levels in comparison to posterior instrumentation techniques are well known. A disadvantage is the necessity of long postoperative immobilization in a body cast or brace due to lack of primary stability. A prospective MRI study was carried out to assess the secondary changes in patients with stenosis and/or herniated disc of the lumbar spinal canal. The study covered 100 patients who had low back and leg pain due to such processes.
