Refractory variceal bleeding is associated with high mortality in patients with chronic liver disease. A fully-covered self-expanding metal stent (SEMS) has been reported to have excellent rates of technical success and initial bleeding control; however, studies to date are small and limited to Europe and Asia. Our aim was to evaluate the efficacy and safety of this SEMS for control of refractory variceal bleeding (VB).

Article Synopsis
  • This study looked into how well two medications, ustekinumab (UST) and vedolizumab (VDZ), work as third-line treatments for patients with Crohn's disease who haven’t responded well to other medications.
  • Researchers included 204 patients, with 76% receiving VDZ followed by UST, and 24% the opposite. After 16 to 22 weeks, about half of patients in both groups showed a response to the treatment.
  • By week 52, similar response rates (around 86%) were observed, with about a quarter to a third of patients attaining clinical remission, indicating that both medications were similarly effective as third-class therapies. *

A 59-year-old man with refractory Cronkhite-Canada syndrome (CCS) had poor clinical response to high-dose intravenous steroids, azathioprine, total parenteral nutrition and best supportive care. He remained highly symptomatic with abdominal pain, diarrhoea, recurrent sepsis and profound weight loss. Infliximab induction was given as rescue therapy, with marked clinical improvement observed within 3 weeks.

Spondyloarthritis (SpA) may be regarded a family of auto-inflammatory conditions with inflammation focused on the joints. These form part of a wider family of immune-mediated inflammatory diseases, which include inflammatory bowel diseases (IBD). These conditions share common elements of pathophysiology and it is perhaps unsurprising, therefore, that individuals with SpA frequently manifest gastrointestinal inflammation, to which the physician managing the patient with SpA must be alert.
