Background: It is routine practice at many institutions to send surgically excised ganglion cysts for pathologic analysis. This adds significant cost to the procedure with questionable benefit. Paediatric trigger-locked thumb (PT-LT) is a common paediatric thumb condition. It is an acquired flexion deformity of the interphalangeal joint because of a size mismatch between an enlarged flexor pollicis longus tendon and a stenotic oblique pulley. It may present clinically as paediatric locked thumb which is more common or infrequently as paediatric trigger thumb.

Case: We present the case of an 11-year-old girl with spinal muscular atrophy (SMA) type III who suffered an intraoperative venous thromboembolism (VTE), leading to death during bilateral extremity contracture releases for deformity correction.

Conclusion: Given the increasing incidence of VTE in pediatric patients, further consideration should be given to broadening the patient population offerring preoperative deep vein thrombosis screening or thromboprophylaxis. Specifically, patients with previous trauma, neuromuscular disorders, and prolonged immobilization or contractures should receive special attention.
