Suppurative hidradenitis is a chronic inflammatory process that involves apocrine glands of the axial, inguinal and perianal region. We present a male patient of 47 years old who was admitted to our hospital with a complex perianal fistula with not a good evolution after surgery. Then, a large resection of the gluteus was performed, because of a big lesion extended to the perianal region.
