Emphysematous pyelonephritis (EPN) is a severe disease of the renal system in which gas formed by facultative anaerobe microorganisms accumulates, being the most representative causative agent. A series of conditions foster its development including uncontrolled diabetes mellitus, diabetic nephropathy and obstructive uropathy. In the context of infection by SARS-CoV-2, the debate arises as to whether these are classified as spontaneous or secondary, since the multiple mechanisms of pulmonary structural damage that COVID-19 entails condition lung fragility in a patient in short time. For the above, we presents the case of a 36-year-old female patient with COVID-19 complicated with pneumomediastinum and subcutaneous emphysema in order to illustrate and discuss these complications.

Background: The SARS-CoV-2 virus, which emerged in late 2019 in Wuhan province, China, has shown a wide spectrum of manifestations, some of which it shares with some autoimmune disorders. Until now, the association of SARS-CoV-2 with immune thrombocytopenia (ITP) is poorly understood, since the mechanisms that explain the complex relationship between these two prothrombotic states are still unknown.
Clinical Case: It is presented the case of a 66-year-old woman, with no chronic disease history, who arrived at the Emergency Department due to respiratory symptoms.
