Background: Although typically associated with a favorable prognosis, a minority of human papillomavirus (HPV)-related (+) oropharyngeal cancers recur after chemoradiation. We postulated that a minor HPV-negative tumor subfraction may be responsible for recurrences of HPV+ oropharyngeal cancer.

Methods: Paired untreated primary and recurrent tumor specimens were identified for 37 patients with oropharyngeal cancer who received definitive chemoradiotherapy at our institution.

Article Synopsis
  • The study investigates why HNSCC cells with mutant p53 in the cytoplasm (p53(mut_c)) show more resistance to cisplatin (CDDP) compared to those with mutant p53 in the nucleus (p53(mut_n)).
  • Researchers used various techniques to evaluate genes associated with this drug resistance, finding that certain transporters (ABCC2 and ABCG2) were upregulated in resistant cells and linked to enhanced survival against CDDP.
  • The findings suggest that p53(mut_c) contributes to a stem cell-like trait in HNSCC cells, leading to increased drug resistance through mechanisms like higher levels of glutathione and metabolic activity.

We present a passive, miniature check valve which can be manufactured using standard techniques ideal for low-cost, disposable systems used in medical devices and other applications. The body of the valve consists of a hollow cylindrical core, closed at one end, with a side port and a cylindrical elastomeric sleeve placed over the core body, covering the side port. The pressure required for initial opening of the valve, referred to as cracking pressure, can be adjusted, and depends predominantly on the valve core outer diameter, the sleeve inner diameter, the sleeve wall thickness, and the sleeve material's modulus of elasticity.

Background: The purpose of this study was to examine the predictive value of hypoxia-inducible factor (HIF)-1α, carbonic anhydrase (CA)-IX, glucose transporter (GLUT)-1, cyclooxygenase (COX)-2, Ki-67, and erythropoietin receptor (EPOR) as immunohistochemical markers for determining the optimal treatment modality for early stage laryngeal carcinoma.

Methods: Tissue samples from 42 early stage laryngeal carcinomas treated with radiotherapy alone were analyzed immunohistochemically for the expression of 6 markers. The Kaplan-Meier method, univariate and multivariate analyses, and the Cox proportional hazards model were used to analyze the associations between patient and tumor characteristics and immunohistochemical results, and locoregional control.

Article Synopsis
  • The biopsychosocial model has struggled to gain traction in research despite its intuitive appeal, partly due to the lack of a clear mechanism that connects biological, psychological, and social processes.
  • Negative feedback might serve as this missing mechanism, influencing neurotransmitter regulation and important psychological aspects like emotional regulation and therapist-client relationships.
  • Perceptual Control Theory (PCT) offers a functional framework that integrates these processes, suggesting that it could unify various research fields and enhance our understanding of how brain functions relate to behavior and environment.

Background: As the role of palliative care (PC) has yet to be clearly defined in patients with heart failure (HF), such patients may face barriers regarding PC referral. In order to maximally meet the needs of HF patients, it is necessary to understand how they compare to the classic PC population: patients with cancer.

Objective: To characterize the unresolved symptom and treatment needs with which patients with HF and those with cancer present to PC.

Article Synopsis
  • The study looked at if adding special tests could help doctors predict cancer comeback after treatment in patients with HPV-related throat cancer.
  • Researchers examined 198 patients to see how certain factors, like tumor and node characteristics, affected the risk of cancer returning.
  • They found that for most HPV-positive patients, only the severity of their cancer stage and a bit of smoking affected the chances of cancer returning, while other factors didn't matter as much.

An important challenge in comparative effectiveness research is the lack of infrastructure to support pragmatic clinical trials, which compare interventions in usual practice settings and subjects. These trials present challenges that differ from those of classical efficacy trials, which are conducted under ideal circumstances, in patients selected for their suitability, and with highly controlled protocols. In 2012, we launched a 1-year learning network to identify high-priority pragmatic clinical trials and to deploy research infrastructure through the NIH Clinical and Translational Science Awards Consortium that could be used to launch and sustain them.

Background Context: Postoperative vision loss complicates an estimated 1 in 1,100 prone spine surgical cases. This complication has been attributed to ischemic optic neuropathy, with one proposed reason being perioperative elevations in intraocular pressure (IOP). Previous research has studied the effects of table inclination on IOP in awake volunteers; however, the effects in spine surgery patients have not been investigated for reverse Trendelenburg positioning using a prospective, randomized controlled study design.

Cervical cancer is responsible for 10-15% of cancer-related deaths in women worldwide. The aetiological role of infection with high-risk human papilloma viruses (HPVs) in cervical carcinomas is well established. Previous studies have also implicated somatic mutations in PIK3CA, PTEN, TP53, STK11 and KRAS as well as several copy-number alterations in the pathogenesis of cervical carcinomas.

Wnt signaling is one of the key oncogenic pathways in multiple cancers, and targeting this pathway is an attractive therapeutic approach. However, therapeutic success has been limited because of the lack of therapeutic agents for targets in the Wnt pathway and the lack of a defined patient population that would be sensitive to a Wnt inhibitor. We developed a screen for small molecules that block Wnt secretion.
