In two studies, the authors evaluated the impact of psychiatric disorders on medical care utilization in a primary care setting. In the first study, 526 consecutive patients in a teaching hospital primary care practice completed the 18-item RAND Mental Health Inventory to identify clinically significant depression and/or anxiety and a questionnaire about the use of psychiatric treatment and psychoactive medications. The medical utilization of those patients defined as depressed and/ or anxious was compared with those defined as not depressed and/or anxious.

The syndrome non-familial acro-osteolysis features bone reabsorption of the digits in association with short stature. Osteoporosis is common in this disorder, but no successful treatment has been previously reported. We report a case of non-familial acro-osteolysis complicated by multiple fractures in which treatment with antiresorptive therapy improved both biochemical markers of bone turnover and bone mineral density but had no effect on the acro-osteolysis.

Because previous studies of high-dose methotrexate usage have demonstrated an effect on bone formation and resorption, this study was done to determine whether long-term, low-dose use of methotrexate for the treatment of rheumatoid arthritis causes bone loss. Bone mineral density (BMD) of the lumbar spine and hip was measured in 10 Caucasian postmenopausal women who had never received methotrexate and 10 Caucasian postmenopausal women who had received the drug for 3 or more years. Systemic mastocytosis has been defined as an abnormal increase in mast cells in tissues other than the skin. We report an unusual case of systemic mastocytosis that was manifested by osteopenia and osteonecrosis, and was associated with pancreatic adenocarcinoma. Our experience suggests that osteopenia and compression fractures may be the presenting features of mastocytosis and that a careful search for a malignancy is warranted in these patients.

We report the results of investigating a pregnant baboon that developed hypertension, proteinuria, and oedema in late gestation. Although the clinical presentation suggested a diagnosis of pre-eclampsia, the evolution of her clinical signs and results of a renal biopsy performed 3 weeks after delivery suggested that glomerulonephritis was the underlying disease.

The diagnosis was missed in part because of repeated negative assays for glomerular basement membrane antibodies. Many medical and neurologic disorders cause psychosis with delusions. The pathophysiology of this final common pathway may shed light on idiopathic psychotic disorders such as schizophrenia.
