This case report documents an uncommon cause of bilateral diaphragmatic paralysis resulting from phrenic nerve injury during cervical chiropractic manipulation. Several months after the initial injury, our patient remains short of breath and has difficulty breathing in the supine position. Other causes of diaphragmatic paralysis and phrenic nerve injury are reviewed.

The repair of injured epithelium involves a complex interaction between epithelial cells and the underlying extracellular matrix. We studied the effects of sublethal concentrations of cigarette smoke extract (CSE) and two volatile components of cigarette smoke, acetaldehyde and acrolein, on bovine bronchial epithelial cell (BBEC) attachment and migration in vitro. After short-term exposure (2 and 6 h) to CSE, BBEC attachment to fibronectin-coated dishes was decreased, and migration to fibronectin was unchanged.

Purpose: A prospective study to determine the usefulness of quantitative bacterial cultures of fluid obtained via fiberoptic bronchoscopy and bronchoalveolar lavage as an aid in the diagnosis of bacterial pneumonia.

Patients And Methods: All patients undergoing fiberoptic bronchoscopy with bronchoalveolar lavage during a 6 1/2-month period. Presence of pneumonia was determined using clinical, radiographic, laboratory, and histologic data.
