We study errors in organizations to understand and ideally prevent them from reoccurring. In this study we examine mistakes made as an oil company adopted new technology to access untapped reserves. We find that a pre-existing error management culture (EMC) dominated in the organization while error prevention measures were deficient.

A series of CeFe(1-x)Co(x)AsO oxyarsenide compounds with Co doping on iron sites (x = 0-0.2) have been synthesized by a solid state reaction method. A microstrain gauge was used to quantify strain and digital calipers were used to assess nerve excursion in 4, fresh, intact cadavers. Data analysis of noncontinuous motion trials showed that the median nerve tension test caused a maximum summative strain in the median nerve at the carpal tunnel of 7.
