Aim: Judgment of factors concerning the indication for the implantation of an artificial disc endoprosthesis, the rating of this operation and biomechanical considerations.
Method: Presentation of the indication and the value of a new operative concept in comparison to a so-called dynamic stabilization procedure (Dynesys' method). Indications for the operation are: unsuccessful, orthopedic conservative treatment for > 6 months, segmental pain, age < 45 years, evidence of an MRI demonstrable mono- or bisegmental disc degeneration with or without disc prolabation, exclusion of psychogenic disease and positive preoperative, diagnostic measures such as facet joint infiltration and discography.

Method: 35 patients with 11 bi- and 24 monosegmental implanted disc endoprosthesis were observed over an average follow-up of 14.7 months.

We report the case of a 29-year-old patient with an uncommon handgun accident. A gun was resting in his pocket when an accidental shot was fired. The projectile penetrated the skin of the thigh, travelled through the quadriceps muscle and entered the articular cavity at the upper pole of the suprapatellar recess.
View Article and Find Full Text PDFA retrospective report on experience with patients with hemorrhagic necrotizing pancreatitis treated surgically with the new therapeutic concept of programmed lavage, so-called, is presented. With the exception of one fatality, using an unchanged intensive care therapy program, 7 patients were successfully treated with the new concept. Since the results obtained with the therapeutic concept used prior to the technique of programmed lavage, as also reports in the international literature, are associated with a very poor prognosis, it is suggested that this new treatment concept be employed with the aim of improving the prognosis.
