Article Synopsis
  • - This study examined the diagnosis and treatment of valvular heart disease (VHD) in Korea, focusing on adult patients with moderate to severe cases through echocardiography in 45 hospitals.
  • - Out of 4,094 patients, about 1,482 had severe VHD, with varied rates of intervention depending on the type of valve issue, highlighting significant in-hospital mortality, especially for secondary severe mitral regurgitation (9.0%).
  • - The findings reveal a 5.4% overall in-hospital mortality rate, with lower mortality in patients who underwent surgical or transcatheter interventions compared to those receiving conservative treatment, suggesting the need for improved VHD management strategies in the future.

Article Synopsis
  • This study investigates the link between lipoprotein(a) levels and the risk of developing severe degenerative aortic stenosis (AS), which can lead to aortic valve replacement (AVR).
  • Among 44,742 patients followed for an average of 6.8 years, those with Lp(a) levels greater than 100 mg/dL had a significantly higher risk for severe AS and needing AVR compared to those with lower levels.
  • The findings suggest that high Lp(a) levels specifically increase the risk for severe degenerative AS, but this association does not apply to other forms of AS, like severe bicuspid or rheumatic AS.

Article Synopsis
  • The study evaluated the effectiveness of using 3D transesophageal echocardiography to determine balloon size for percutaneous mitral balloon commissurotomy (PMBC) in patients with mitral stenosis (MS).
  • Traditional methods of selecting balloon size rely on a patient's height or body surface area, which may not be as accurate as the new approach based on mitral valve measurements.
  • Results showed that using optimized formulas for balloon size selection led to different outcomes in groups with successful PMBC, residual MS, or significant mitral regurgitation (MR), suggesting that 3D measurements can improve treatment efficacy.

Article Synopsis
  • In a study of patients aged 60 and older with patent foramen ovale (PFO) and cryptogenic stroke from South Korea, researchers aimed to determine if PFO closure helps reduce the risk of recurrent strokes compared to medical therapy alone.
  • The study included 437 patients, with 37% undergoing PFO closure; results showed that PFO closure significantly lowered the risk of subsequent ischemic strokes or TIAs over a follow-up period of about 3.9 years.
  • The findings suggest that elderly patients with high-risk PFO have a substantial recurrence rate of strokes, which PFO closure can effectively reduce.

Article Synopsis
  • The study focuses on left atrial thrombus (LAT) formation in patients with atrial fibrillation (AF) already on anticoagulation therapy for over 4 weeks, highlighting its link to thromboembolic events.
  • The research analyzed 1,122 patients and found that LAT was present in 5.3% of them, with factors like age, type of AF, CHADS-VASc score, left ventricular ejection fraction, and atrial enlargement influencing its presence.
  • A predictive model combining these clinical and echocardiographic factors showed strong performance, indicating it could help identify patients who might need further imaging before treatment interventions.

Article Synopsis
  • IgG4-related effusive constrictive pericarditis (ECP) is a rare but serious condition that can cause heart issues like cardiac tamponade and heart failure, primarily treatable with glucocorticoids to reduce inflammation and fibrosis.
  • A recent case of a 66-year-old woman demonstrated the effectiveness of fluorine-18 fluorodeoxyglucose (F-FDG) PET/CT scanning in diagnosing and monitoring treatment response for this condition, showing increased F-FDG uptake in the pericardium.
  • After treatment with prednisolone and colchicine, the patient exhibited significant improvement in symptoms and imaging, allowing for successful tapering of medications.

Background: Valvular heart disease (VHD) is a common cause of cardiovascular morbidity and mortality worldwide; however, its epidemiological profile in Korea requires elucidation.

Methods: In this nationwide retrospective cohort study from the Korean valve survey, which collected clinical and echocardiographic data on VHD from 45 medical centers, we identified 4,089 patients with VHD between September and October 2019.

Results: The aortic valve was the most commonly affected valve (n = 1,956 [47.

Myocardial calcification can develop owing to several conditions. It is a rare complication following sepsis and renal failure. We report a case of rapid development of left ventricular mid-wall calcification observed using CT and cardiac MRI in a patient after recovery from sepsis and acute renal failure.

Although tricuspid regurgitation (TR) is a general medical issue with growing prevalence and socioeconomic burden, most clinicians have not paid much attention to TR in the past. Several problems of TR have been pointed out in clinical practice, which include: ambiguous clinical manifestations and the difficulty in initial detection, limitations in generally used diagnostic tools, the absence of objective criterion for therapeutic intervention, high operative morbidity and mortality, and lack of long-term clinical data after the intervention for TR. Therefore, patients with TR usually visit clinicians at a much-advanced state, and this delay gives a major dilemma in clinical decision-making in a routine clinical practice.

Bicuspid aortic valve (BAV), a common congenital anomaly with various morphological phenotypes, is also characterised by marked heterogeneity in clinical presentations including clinically silent condition with mild valvulo-aortopathy, progressive valvulopathy and complex valvulo-aortopathy with shorter life expectancy. The clinical importance of using a general and unified nosology for BAV is well-accepted by opinion leaders and an international consensus statement has been recently published, which will serve as an important scientific platform for BAV. This review describes the current knowledge of BAV based on clinical studies, addresses several unresolved issues requiring investigators' attention and highlights the necessity of prospective studies with a very long follow-up duration for better appreciation of BAV-associated valvulo-aortopathy.

Background And Objectives: The prognostic value of left atrial (LA) function in terms of long-term clinical outcomes after mitral regurgitation (MR) surgery remains unclear. Therefore, we investigated the impact of preoperative LA global longitudinal strain (LAGLS) on the long-term postoperative clinical outcomes in chronic severe MR patients who underwent mitral valve (MV) repair surgery.

Methods: From January 2012 to December 2017, we analyzed 338 patients (mean age, 51.

Dasatinib, a tyrosine kinase inhibitor (TKI), induces pulmonary hypertension (PH) in patients with chronic myeloid leukemia (CML). However, information on other TKIs is limited. We retrospectively analyzed PH prevalence by reviewing transthoracic echocardiography (TTE) findings in a population of Korean CML patients treated with TKI at a single hospital between 2003 and 2020.

Unlike the left ventricle (LV), the left atrium (LA) has a thin-walled structure and has been regarded as a simple conduit chamber. However, the unique function of the LA to modulate LV filling has recently drawn much attention. Because LA structure and function are directly influenced by the LV filling pressure, LA assessment is an essential step in the diagnosis of diastolic dysfunction that can help predict new-onset atrial fibrillation, assess the risk of further embolic events, and identify high-risk patients for adverse cardiovascular events.

Background: Left atrial (LA) strain is a novel parameter of LA function. However, its reference value has not been established, and the determining factors for LA strain remain elusive. We aimed to present LA strain with reservoir, conduit, and contractile components and associated parameters in a large-sized group of healthy individuals.
