The medical records of 967 patients treated with a modified neck dissection were carefully reviewed, and the data were collected and statistically analyzed. For a primary tumor in the oral cavity or oropharynx, a supraomohyoid neck dissection was adequate treatment for the neck that was both clinically staged N0 or N1 and pathologically staged N1 without evidence of extracapsular invasion. For primary tumors in the larynx and hypopharynx, an elective bilateral anterior neck dissection is considered proper treatment if the nodes are not multiple or if connective tissue disease is not present.

A cohort of 6875 homosexual men, initially seen at the San Francisco City Clinic between 1978 and 1980, were studied to determine the incidence and prevalence of the acquired immunodeficiency syndrome, related conditions, and infection with the human T-lymphotropic virus, type III/lymphadenopathy-associated virus (HTLV-III/LAV). By December 1984, 2.4% of the men had the syndrome; mortality attributable to the syndrome in 1984 was 600/100 000.

Eleven cases of metastases to the intrinsic larynx are reported. This number constitutes the largest collection from a single institution in the literature and brings the total recorded cases to 83. Cutaneous melanomas are the preponderant primaries, followed by renal cell carcinomas, and carcinomas of the breast and lung, respectively.

Tissue capillarity in the gastrocnemius and myoglobin concentrations in the gastrocnemius and heart were determined for hatchling Canada geese, Branta canadensis, following incubation of the embryos under either normoxic (Po2 = 120 torr) or hypoxic (Po2 = 94 torr) conditions. Similar observations were made on a limited number of hatchling bar-headed geese, Anser indicus, a species native to high altitude. Capillary densities were higher and diffusion distances shorter in the hypoxic Canada geese and the bar-headed geese than in the normoxic Canada geese.

Tissue capillarity, diffusion distances and myoglobin concentrations in the anterior latissimus dorsi (ALD), consisting entirely of slow tonic fibers, the posterior latissimus dorsi (PLD), consisting entirely of twitch fibers, and the gastrocnemius (GST), consisting of both twitch and tonic fibers, were determined for the domestic chicken throughout maturation. Increases in muscle mass for the three muscles are due to fiber hypertrophy and are quantified as changes in the average cross-sectional area (FCSA) of the fibers. Capillary densities were highest in the newly hatched chicks: ALD = 3967 +/- 888 caps/mm2, PLD = 8360 +/- 1078 caps/mm2, and GST = 8360 +/- 8360 +/- 1283 caps/mm2; and decreased in proportion to the increase in FCSA, to adult values of ALD = 450 +/- 50, PLD = 120 +/- 20 and GST = 267 +/- 33 caps/mm2.

Seventy-seven patients with primary malignant parotid tumors were studied and the various factors relative to their disease, as well as their treatment, were analyzed. Because of the retrospective nature of this study, no attempt was made to compare surgery alone with surgery and postoperative radiation, but it was interesting to reflect on the relative and absolute indications for the use of combined treatment versus surgery alone. The above-clavicle control in this group of patients was excellent, and the relatively low incidence of distant metastases reflect that optimal local control was closely correlated with survival.

A clinico-pathologic study of 12 patients, each harboring a hitherto not delineated adenocarcinoma of salivary origin is presented. The authors have designated this histologically unique carcinoma as 'terminal duct adenocarcinoma' in deference not only to its light-optic appearance, but also to a putative origin from the reserve cells (epithelial and myoepithelial) of the intercalated duct. The tumors' local invasive properties with extension into nerves and adjacent bone suggest their biologic behavior is like that of adenoid cystic carcinomas.

This study was undertaken to determine what optimal levels of suction pressure were necessary to provide good drainage volume and obliteration of any dead space and also to determine the prevalence of clotting and complications secondary to various levels of suction pressure. The patients were grouped by their degree of nutritional depletion, prior radiation exposure, the types of surgical procedures undergone, and the results of tests using four levels of suction pressure. Three of the suction pressure values were obtained with a wall suction and one was obtained using a portable closed system.

Laryngeal carcinoma usually occurs in men between the ages of 50 and 70. In general, epithelial malignancies other than those involving the skin are rare in the young adult. Those squamous carcinomas arising in the lip, tongue, and tonsil appear to have a poorer prognosis than in the older adult.

The current operating budget for American eyebanks is more than $5 million per year, and an increasing portion of this expense is being paid by third-party payers. American eyebanks were surveyed to determine what a reasonable cost per transplant tissue might be and to see if recommendations could be made that would help optimize cost-effectiveness in American eyebanking. Most well-run eyebanks needed between $222 to $464 to process each donor cornea for transplantation.

The records of 61 patients with a diagnosis of squamous carcinoma of the lower alveolar ridge were reviewed. The overall two-year survival was 67% with a local or regional failure of 5%. The majority of patients were treated with surgery initially with an excellent local control of 98%, if the patient who died in the postoperative period and the patient lost to follow-up are excluded.

Forty-seven patients with very advanced cancer of the pyriform sinus were treated with surgery and immediate postoperative radiation in a planned sequential manner. Eradication of the cancer above the clavicles was obtained in 81% of the patients, with a treatment mortality of 8%. Most of the patients' tumors were resected with primary closure, which obviated the need for complicated reconstructive procedures.
