Background: Therapeutic plasma exchange (TPE) is the primary intervention for treating symptomatic hyperviscosity from hypergammaglobulinemia, yet its efficacy for treating hyperviscosity related to hyperfibrinogenemia is unclear.

Objective: Define the safety and efficacy of TPE for critically ill COVID-19 patients with elevated blood viscosity from hyperfibrinogenemia.

Method: A prospective, randomized controlled trial in critically ill COVID-19 patients at a single US healthcare system.

The application of flexible risers has led to increased production of fluid contents in the marine industry. This paper presents the design challenges of a flexible riser subjected to internal pressure under deep-water conditions, at a water depth of 2000 m. Parametric variations with extensive dynamic analysis were carried out.

Contextual cuing survives an interruption from an endogenous cue for attention.

Three experiments explored how the repetition of a visual search display guides search during contextual cuing under conditions in which the search process is interrupted by an instructional (endogenous) cue for attention. In Experiment 1, participants readily learned about repeated configurations of visual search, before being presented with an endogenous cue for attention towards the target on every trial. Participants used this cue to improve search times, but the repeated contexts continued to guide attention.

Ultraviolet radiation thin film dosimetry: A review of properties and applications.

Spectroradiometry, radiometry, and dosimetry are employed for the measurement of ultraviolet radiation (UVR) irradiance and non-ionizing exposure. Different types of UVR dosimeter have been developed for measuring personal and environmental UVR exposures since film dosimetry was pioneered in the 1970s. An important type of dosimeter is the thin film variant, which contains materials that undergo changes in optical absorbance when exposed to UVR.

Insights Towards Trauma-Informed Nursing Supervision: An Integrative Literature Review and Thematic Analysis.

Trauma-informed care (TIC) is an approach to healthcare practice that recognises the pervasiveness of trauma, and the deep and life-shaping impact this has on a person. The literature recognises the negative consequences of trauma both to the people who experience it, and the nurses who provide care for them. Professional supervision is an integral element of workforce wellbeing and practice development, and a largely unexplored avenue of support for those who deliver TIC.

Prenatal Care Clinician Preferences Among Patients With Spanish-Preferred Language.

Objective: To measure what patients with Spanish language preference and limited English proficiency value most when selecting a prenatal care clinician.

Methods: A discrete choice experiment was administered at two large academic medical centers in Boston, Massachusetts. Participants were identified by electronic medical record, had preferred Spanish language and self-identified limited English proficiency, and either were pregnant with a completed fetal anatomy scan or had given birth within the past 12 months at the time of the study.

The mitigating effect of street trees, urban flora, and the suburban environment on seasonal peak UV indices: A case study from Brisbane, Australia.

Tree shade, particularly shade that obscures direct sunlight near peak periods of midday solar exposure can have a pronounced effect on potentially harmful ultraviolet radiation, and in turn, strongly influence the maximum daily UV index (UVI). In this study, the seasonal influence of tree shade on the UVI is evaluated from 210 hemispherical sky view images collected alongside public walkways and footpaths from 10 residential Brisbane suburbs. The effective sidewalk UV index is calculated underneath planted tree canopies, adjacent residential gardens, buildings and background tree species.

Bioprocess monitoring applications of an innovative ATR-FTIR spectroscopy platform.

Pharmaceutical manufacturing is reliant upon bioprocessing approaches to generate the range of therapeutic products that are available today. The high cost of production, susceptibility to process failure, and requirement to achieve consistent, high-quality product means that process monitoring is paramount during manufacturing. Process analytic technologies (PAT) are key to ensuring high quality product is produced at all stages of development.

The impact of sickle cell disease and its treatment on ovarian reserve in reproductive-aged Black women.

We compared serum anti-Mullerian hormone (AMH) levels in women with sickle cell disease (SCD) (n = 152) to those of Black comparison women (n = 128) between the ages of 20 and 45 years and evaluated the impact of hydroxyurea (HU) and iron overload on ovarian reserve in those with SCD. SCD treatment was abstracted from medical records. Linear regression models were fit to examine the relationship between log(AMH) and SCD, adjusting for age.

Predicting Everyday Critical Thinking: A Review of Critical Thinking Assessments.

Our ability to think critically and our disposition to do so can have major implications for our everyday lives. Research across the globe has shown the impact of critical thinking on decisions about our health, politics, relationships, finances, consumer purchases, education, work, and more. This chapter will review some of that research.

Palliative care research utilising intersectionality: a scoping review.

Background: Access to palliative care is recognised as a human right, yet clear disparities exist. There have been recent appeals to examine people's contexts and interactions with social systems which for many, adversely influence their utilisation of palliative care. Intersectionality provides a way to understand these drivers of inequity and ultimately advocate for change.

Communication Preferences During Pregnancy Care Among Patients With Primary Spanish Language: A Scoping Review.

Background: Qualified language service providers (QLSPs)-professional interpreters or multilingual clinicians certified to provide care in another language-are critical to ensuring meaningful language access for patients. Designing patient-centered systems for language access could improve quality of pregnancy care.

Objective: We synthesized and identified gaps in knowledge about communication preferences during pregnancy care among patients with Spanish primary language.

A spectroscopic liquid biopsy for the earlier detection of multiple cancer types.

Background: A rapid, low-cost blood test that can be applied to reliably detect multiple different cancer types would be transformational.

Methods: In this large-scale discovery study (n = 2092 patients) we applied the Dxcover® Cancer Liquid Biopsy to examine eight different cancers. The test uses Fourier transform infrared (FTIR) spectroscopy and machine-learning algorithms to detect cancer.

Experiences and Communication Preferences in Pregnancy Care Among Patients With a Spanish Language Preference: A Qualitative Study.

Objective: To explore Spanish-speaking patients' experiences and preferences regarding communication during pregnancy care with specific attention to language barriers.

Methods: Patients with a Spanish language preference who gave birth between July 2022 and February 2023 at an academic medical center were invited to participate in focus groups. Focus groups were held over Zoom, audio-recorded, transcribed in Spanish, translated into English, and reviewed for translation accuracy.

Background: Amyloid plaques and neurofibrillary tangles, the molecular lesions that characterize Alzheimer's disease (AD) and other forms of dementia, are emerging as determinants of proteinopathies 'beyond the brain'. This study aims to establish tau's putative pathophysiological mechanistic roles and potential future therapeutic targeting of tau in heart failure (HF).

Methods And Results: A mouse model of tauopathy and human myocardial and brain tissue from patients with HF, AD, and controls was employed in this study.

Optimizing the Seasonal Shade and Ultraviolet Protection of a Suburban Playground by Utilization of the Playground Shade Index.

The Playground Shade Index (PSI) is introduced as a design metric for assessing the shade and solar ultraviolet (UV) protection provided by eight different cloth weave shade structures currently utilized in public playgrounds across a regional city located in southern Queensland, Australia. The PSI, expressing the ratio of available ambient solar UV exposure relative to the UV exposure received by a shaded 184 m playground surface over a whole day in midsummer and midwinter, is used to determine the best design and aspect of all eight existing structures. Unlike subjective shade audits that may only consider the type of shade structure, the number and characteristics of surrounding trees, and surface materials available at a given site, the PSI considers, in addition to previous factors, the available sky fraction and the direct solar UV contribution underneath the shade structure over a full day.

Are mental health nurses meeting the requirements of second health professionals in presenting opinions to the court?

WHAT IS KNOWN ON THE SUBJECT?: Mental health legislation is currently under review following recommendations for repeal and replacement. Through the statutory role of the second health professional, mental health nurses are in a strong position to offer a unique clinical perspective, engagement and advocacy for tangata whaiora (person seeking wellness) and whānau. Māori are more likely to be subject to a compulsory treatment order than non-Māori.

Identifying and prioritising the key components of a Quality Improvement Network for allied health professionals and psychological therapists: a group concept mapping project.

Introduction: Despite growing enthusiasm for quality improvement (QI), the complexities of modern healthcare continue to create gaps in our ability to consistently deliver the most effective and efficient care for patients, and improvement activities often fail to achieve widespread uptake even when there is robust evidence of their benefits.

Methods: We undertook a novel, mixed methods evaluation and planning project using group concept mapping (GCM) methodology to identify and prioritise the ways in which our recently established Quality Improvement Network (QIN) could support allied health professionals, psychological therapists and administrative staff in their daily work to improve patient outcomes and experience. Mid-level leaders across our therapy services department contributed towards a statement generation activity and individually sorted these statements into themes.

Archival data were collected from a sample of 636 persons convicted of capital murder and sentenced to death in the State of California.

Environmental and Anthropogenic Factors Shape the Skin Bacterial Communities of a Semi-Arid Amphibian Species.

The amphibian skin microbiome is important in maintaining host health, but is vulnerable to perturbation from changes in biotic and abiotic conditions. Anthropogenic habitat disturbance and emerging infectious diseases are both potential disrupters of the skin microbiome, in addition to being major drivers of amphibian decline globally. We investigated how host environment (hydrology, habitat disturbance), pathogen presence, and host biology (life stage) impact the skin microbiome of wild Dhofar toads (Duttaphrynus dhufarensis) in Oman.

The Playground Shade Index: A New Design Metric for Measuring Shade and Seasonal Ultraviolet Protection Characteristics of Parks and Playgrounds.

Current shading strategies used to protect outdoor playgrounds from harmful solar radiation include the placement of artificial cloth weaves or permanent roofing over a playground site, planting trees in proximity to playground equipment, and using vegetation or surface texture variations to cool playground surfaces. How and where an artificial shade structure is placed or a tree is planted to maximize the shade protection over specific playground areas, requires careful assessment of local seasonal sun exposure patterns. The Playground Shade Index (PSI) is introduced here as a design metric to enable shade and solar ultraviolet exposure patterns to be derived in an outdoor space using conventional aerial views of suburban park maps.

A scoping review of clinical practice guidelines for the diagnosis of primary spontaneous pneumothorax.

Introduction/background: Primary spontaneous pneumothorax (PSP) is characterised by the onset of pneumothorax with no evidence of trauma or associated co-morbidities. Several clinical practice guidelines (CPGs) have been published regarding the management of PSP. Inconsistency in imaging protocols across clinics globally may indicate variability in the recommendations within these guidelines.

Predicting probability of tolerating discrete amounts of peanut protein in allergic children using epitope-specific IgE antibody profiling.

Background: IgE-epitope profiling can accurately diagnose clinical peanut allergy.

Objective: We sought to determine whether sequential (linear) epitope-specific IgE (ses-IgE) profiling can provide probabilities of tolerating discrete doses of peanut protein in allergic subjects undergoing double-blind, placebo-controlled food challenges utilizing PRACTALL dosing.

Methods: Sixty four ses-IgE antibodies were quantified in blood samples using a bead-based epitope assay.

Consideration of metabolomics and transcriptomics data in the context of using avian embryos for toxicity testing.

Early-life stage (ELS) avian toxicity tests have been proposed as a more ethical alternative to traditional standardized tests with adult birds. At the same time, 'omics approaches are gaining traction in the field of avian toxicology, but little has been done to characterize the metabolome and transcriptome at different life stages. The present study uses 'omics data from toxicity tests of 8 environmental chemicals in ELS and adult Japanese quail (Coturnix japonica) to address this data gap.

A flexible mixed-data model applied to claims data for post-market surveillance of prescription drug safety behavior.

We develop a new modeling framework for jointly modeling first prescription times and the presence of risk-mitigating behavior for prescription drugs using real-world data. We are interested in active surveillance of clinical quality improvement programs, especially for drugs which enter the market under an FDA-mandated Risk Evaluation and Mitigation Strategy (REMS). Our modeling framework attempts to jointly model two important aspects of prescribing, the time between a drug's initial marketing and a patient's first prescription of that drug, and the presence of risk-mitigating behavior at the first prescription.
