The COVID-19 pandemic has required many pharmacy programs to increase their utilization of technology or shift the course of delivery entirely online. Delivery in this setting has exposed areas in the use of technology where pharmacy programs need to improve (such as staff and student training). Two apparently unrelated families with a history of leukodystrophy associated with adrenal insufficiency are presented. Only about 20 cases of this syndrome have been reported until now. It was first described by Siemerling and Creutzfeldt; therefore we propose the designation Siemerling-Creutzfeldt disease.

A secondary Cushing's syndrome developed in a 38-year-old woman from an ACTH secreting phaeochromocytoma. Large amounts of ACTH were demonstrated in the tumour by radioimmunological tests. A remarkable feature was that clinical and biochemical signs of increased catecholamine secretion completely regressed with time and the adrenals were stimulated at intervals by the ectopic ACTH secretion, steroid excretion falling to normal in between.

Isolated glucocorticoid insufficiency combined with intact secretion of aldosterone and high plasma level of ACTH are characteristic. Blood glucose values are elevated reflecting a diabetic metabolism lead to a suppressive effect of increased STH-blood levels. In general it can be stated, that a stress situation is followed by an increase of STH-secretion despite of elevated blood glucose concentrations.
