The rectus sheath hematoma is a generally rare disease, however the frequency of occurrence has increased with introduction of anticoagulant therapy. It almost always mimics the symptoms of acute abdomen. In 7 out of 14 cases, which we observed, an anticoagulant therapy was administered.

Unstable intertrochanteric fractures of the femur in aged patients primarily require a full weight-bearing stabilization because of the high vital risk due to immobilization. Valgus osteosynthesis with 160 degrees angled plate fixation and more or less medial displacement of the shaft fragment has produced good clinical results. The very low lethality of only 10% in our patients with an average age of 78.

The supply area of each particular stomach artery was illustrated by the Technovit corrosion technique and X-rays. Despite the extensive skeletonization of the stomach, an adequate blood supply was maintained as a whole, especially the extreme fundus area, solely via the A. gastroepiploica dextra.

In polytraumatized patients, pulmonary contusion complicated by pneumonia is associated with a high risk of fatality. The effects of both contusion and bacterial infection on pulmonary function can be estimated early by measuring the parameters PAPd-PCWP, pulmonary shunt volume and maximal PaO2 following FiO2 = 1.0 for 20 min.

Eleven mongrel dogs were observed for 72 h following a standardized traumatic hemorrhagic shock. The decrease of plasma albumin concentration and blood 125-I-albumin activity with a concomitant increase in lung I-albumin content is possibly caused by capillary leakage. A rise in the extravascular lung water content was measured by a gravimetric postmortem method.

Even after the introduction of radioimmunological methods the question of a cardiac glycoside causing or contributing to the death of a patient can not be answered satisfactorily. By means of a special radioimmunoassay procedure for digoxin as well as for the structurally related methyl- and acetylderivatives we measured the concentrations in human blood and post mortem tissues. We investigated the glycoside contents in the blood of intravenously digitalised (Novodigal) al) patients before and after death.

Fifteen mongrel dogs are subjected to a standardized traumatic hemorrhagic shock. Plasma albumin, measured by J-125-albumin distribution, decreases to 58% of its original level after trauma and hemorrhage. Lung albumin content increases to 53% of the blood level.

Local fibrinolytic activity was studied in normal and hemorrhagic gastric juice and in the gastric wall of men and pigs. This activity was definitely higher in hemorrhagic juice than in normal gastric juice, and its probably released from hemolyzed erythrocytes and mucosal lesions of the gastric wall.

Massive retroperitoneal bleeding in pelvic trauma is a difficult problem in surgery, and surgical treatment is rarely attempted due to the risk involved. Angiographic embolization in this region is an uncomplicated and effective measure that may be performed without general anesthesia and simultaneous with shock treatment. Four patients with massive traumatic hemorrhage in the pelvic region were successfully treated with embolization.

Cervical oesolphagogastrostomy (Kirschner's operation) was performed on 16 patients with carcinoma of the oesophagus. There were five hospital deaths (two due to myocardial infarction). The average hospital stay of the survivals was 22 days.

The individual supply areas of the stomach arteries are clearly shown by the technovit corrosion methods, angiography and measurement of the circulation by radioactive microspheres. All three methods prove that the right gastro-epiploic artery is capable of maintaining an adequate blood supply of the stomach including the critical fundus region.

Animal experiments on 40 Wistar rats show that intraperitoneal application of Varidase can reduce or prevent the formation of adhesions after intra-abdominal interventions. This effect is interpreted as the sequel to a local increase in fibrinolytic activity which leads to the accelerated breakdown of fibrinous pseudo-membranes. The healing of intestinal sutures appears not to be impaired by Varidase.

Hemorrhagic gastric juice contains a higher fibrinolytic activity than normal gastric juice. It is liberated from damaged muco and possibly from hemolytic erythrocytes.
