Incorporating an advance care planning screening tool into routine health assessments with older people.

General practice is arguably the ideal setting to initiate advance care planning (ACP), but there are many barriers. This pilot study was designed to assess the feasibility, acceptability and perceived utility of a nurse-facilitated screening interview to initiate ACP with older patients in general practice. Patients were recruited from four general practices in Sydney, Australia.

Design: A qualitative approach using semistructured telephone interviews. Analysis used an integrative approach.

Low-intensity educational interventions supporting self-management to improve outcomes related to chronic breathlessness: a systematic review.

Chronic breathlessness is debilitating and frightening, often resulting in emergency department presentations with acute-on-chronic breathlessness. Self-management is complex, involving 14 components as identified by the Practical Systematic Review in Self-Management Support (PRISMS). Low-intensity educational interventions that support breathlessness self-management through written/visual educational materials, alongside limited health professional support, are available.
