Measurement of changes in arterial vessel diameter can be used to assess the state of cardiovascular health, but the use of such measurements as biomarkers is contingent upon the accuracy and robustness of the measurement. This work presents a simple algorithm for measuring diameter from B-mode images derived from vascular ultrasound. The algorithm is based upon Gaussian curve fitting and a Viterbi search process.

We tested the hypothesis that the decline in muscle sympathetic activity during and after 8 h of poikilocapnic hypoxia (Hx) was associated with a greater sympathetic baroreflex-mediated responsiveness. In 10 healthy men and women (n=2), we measured beat-to-beat blood pressure (Portapres), carotid artery distension (ultrasonography), heart period, and muscle sympathetic nerve activity (SNA; microneurography) during two baroreflex perturbations using the modified Oxford technique before, during, and after 8 h of hypoxia (84% arterial oxygen saturation). The integrated baroreflex response [change of SNA (DeltaSNA)/change of diastolic blood pressure (DeltaDBP)], mechanical (Deltadiastolic diameter/DeltaDBP), and neural (DeltaSNA/Deltadiastolic diameter) components were estimated at each time point.

To determine whether an approach such as the modified Oxford technique can consistently produce data that reveal the nonlinear nature of the cardiovagal baroreflex and to ascertain whether the model parameters provide unique insight into baroreflex function, we retrospectively examined 91 baroreflex trials (38 subjects, 27 men and 11 women, ages 22-72 yr). The modified Oxford technique (bolus sodium nitroprusside followed by bolus phenylephrine) was used to perturb blood pressure, and the resulting systolic blood pressure-R-R interval responses were plotted and modeled using a linear, a four-parameter symmetric, and a five-parameter asymmetric model. Several issues, such as the effect of data averaging, various approaches to gain estimation, and the predictive value of model parameters, were examined during reflex modeling.

Individuals with chronic fatigue syndrome (CFS) experience a number of somatic complaints including severe, disabling fatigue, and exercise intolerance. We hypothesized that hypovolemia, through its interaction with central hemodynamics, would contribute to the exercise intolerance associated with this disorder. We examined blood volume, peak aerobic power, habitual physical activity, fatigue level, and their interrelations to understand the physiological basis of this disorder.
