Purpose: Distributive stereotactic radiosurgery (dSRS) is a form of fractionation where groups of metastases are treated with a full single-fraction dose on different days. The challenge with dSRS is determining optimal target groupings to maximize the distance between targets treated in the same fraction. This study aimed to develop and validate an accessible optimization technique for distributing brain metastases into optimal treatment fractions using a genetic algorithm.

Introduction: Acute valvular emergencies, a time-sensitive diagnosis, are nearly impossible to diagnose without ultrasound, and missing the diagnosis can significantly impact patient outcomes. Many emergency physicians lack access to echo technicians and may be uncomfortable performing the ultrasound themselves. Approaching the paucity of review articles, none of which are focused for the emergency physician, can be quite daunting, even for those with extensive ultrasound training.

Background: In sub-Saharan Africa, rural areas have lower rates of access to safe drinking water compared to urban areas. We investigated predictors of Escherichia coli contamination in drinking water of rural households in Ghana, Malawi, Mozambique, Niger, Rwanda, Uganda, and Zambia.

Methods: We used a population-based, cluster randomized sampling design to select rural households in each country.

Performing an ultrasound-guided nerve block (UGNB) is now common practice in many emergency departments (EDs) and is considered a core skill according to the American College of Emergency Physicians (ACEP). Nerve blocks are mostly utilized for fractures and laceration repairs, however, these blocks have many other applications. We present a case of utilizing an ulnar UGNB in a patient with flexor tenosynovitis and a history of intravenous drug use (IVDU) when parental opiates proved to be ineffective.

Purpose: Minimizing intermediate dose spill in stereotactic radiosurgery (SRS) for brain treatment is crucial. Intermediate dose spill correlates with the exposure of normal brain tissue to high doses, which increases the risk of radionecrosis. R50%, defined as the volume of the 50% of prescription isodose cloud/planning target volume, is one metric for intermediate dose spill.

Toxins TcdA and TcdB are the main virulence factors of Clostridioides difficile, a leading cause of hospital-acquired diarrhea. Despite their importance, there is a significant knowledge gap of druggable targets for inhibiting toxin production. To address this, we screened nonantibiotic phytochemicals to identify potential chemical genetic probes to discover antivirulence drug targets.

This study describes sources of COVID-19 vaccine information and COVID-19 knowledge among sexual and gender minority (SGM) adults in New York City (NYC). A sample of 986 SGM adults in NYC completed an online survey between June 25 and December 1, 2021. Participants indicated their top three sources of COVID-19 vaccine information from a list of 10 options.

Article Synopsis
  • Dimorphism is a phenomenon where certain fungi can switch between yeast and filamentous forms depending on temperature, which is explored in the study of a new mucoralean fungus isolated from a patient with Burkitt lymphoma.
  • The fungus shows significant changes in shape and structure at body temperature, resembling traditional yeast forms but still having unique features that can help differentiate it from other similar fungi in clinical settings.
  • Accurate and timely diagnosis is crucial for managing mucormycosis, as different fungal species have varied responses to antifungal treatments, highlighting the need to recognize these dimorphic transitions to improve patient outcomes.

Context.—: With increasing availability of immediate patient access to pathology reports, it is imperative that all physicians be equipped to discuss pathology reports with their patients. No validated measures exist to assess how pathology report findings are communicated during patient encounters.

Objectives: Bystander intervention (BI) is a prevention approach commonly used for interpersonal violence, but is less studied for problematic alcohol use (PAU). Domestic graduate and international students' life experiences bring a unique context for the potential application of BI to PAU.

Methods And Measures: We conducted a mixed methods study that consisted of a needs assessment and focus groups at a southeastern university in the U.

Article Synopsis
  • Sexual and gender minority (SGM) individuals face a higher risk of sexual violence (SV), and the reactions they receive when disclosing their experiences significantly affect their mental health.
  • A study found that 83% of SGM survivors disclosed their SV experiences to other SGM individuals, who tended to provide more supportive responses than cisgender/heterosexual recipients.
  • Positive social reactions from cisgender/heterosexual individuals helped reduce depression, while negative reactions from SGM recipients increased depression and PTSD scores, revealing the complex impact of social support on mental health outcomes for SGM survivors.

Article Synopsis
  • The study explored the phases of bystander intervention (BI) specifically for addressing problematic alcohol use (PAU) among college students, using focus groups and interviews to gather data.
  • The Bystander Intervention for Problematic Alcohol Use Model (BIPAUM) consists of five key phases: plan in advance, notice and interpret a sign, decide to intervene, take action, and assess the outcomes, which are all influenced by various barriers and facilitators.
  • The findings suggest that incorporating these phases into intervention programs can effectively address PAU, and further empirical testing of the BIPAUM is recommended to enhance strategies aimed at reducing risky drinking among college students.

Introduction: Compared with previous geographically localized outbreaks of monkeypox (MPOX), the scale of the 2022 global mpox outbreak has been unprecedented, yet the clinical features of this outbreak remain incompletely characterized.

Methods: We identified patients diagnosed with mpox by polymerase chain reaction (PCR; n = 36) from July to September 2022 at a single, tertiary care institution in the USA. Demographics, clinical presentation, infection course, and histopathologic features were reviewed.

Background: Recent ACE research proposed items to assess ACE dimensions, such as the frequency or timing of adverse events, that can be added to the original ACE-Study Questionnaire.

Objective: The goal of our study was to pilot-test the refined ACE-Dimensions Questionnaire (ACE-DQ) to determine its predictive validity and compare scoring approaches.

Participants And Setting: Cross-sectional online survey via MTurk with U.

Introduction: A wide range of descriptive terms have been used for dermoscopic findings in intraepidermal carcinoma (IEC) and the clinical diagnostic accuracy of IEC can be challenging. Furthermore, dermoscopic findings in IEC have only rarely been evaluated in fair-skinned populations.

Objectives: To measure the interobserver agreement between dermatologists for dermoscopic findings in IEC.

Article Synopsis
  • Influenza remains a major cause of death globally, leading to efforts to develop antiviral strategies that have been met with resistance from the virus, including resistance to the new drug baloxavir marboxil.
  • Researchers have leveraged structural insights from endonuclease-substrate complexes to design new inhibitor molecules that specifically target the endonuclease, aiming to minimize the chances of resistance mutations.
  • The team successfully created potent inhibitors effective against both normal and resistant strains of the endonuclease, including macrocyclic versions, and also identified ways to enhance the effectiveness of these cyclic compounds based on structural analysis.

Kink practitioners are marginalized and experience adverse health and social outcomes, which are exacerbated by consent violations. This study aims to understand experiences of reporting consent violations within a kink context. Kink practitioners ( = 2,888) completed a survey focused on consent violations, reporting, and recommendations, with 767 (25.
