Liposomal Bupivacaine Versus Bupivacaine and Dexamethasone Intercostal Nerve Blocks for Robotic Thoracic Surgery: A Randomized Clinical Trial.

Introduction For peripheral nerve blocks, using either the liposomal formulation of bupivacaine or plain bupivacaine with epinephrine and dexamethasone as an adjuvant has been shown to improve postoperative pain scores. In a single-blinded, randomized controlled study of patients undergoing robotic-assisted thoracoscopic surgery, we determined if bupivacaine with epinephrine and dexamethasone was noninferior to liposomal bupivacaine mixed with plain bupivacaine when administered intraoperatively as an intercostal nerve block (INB). Methods A total of 34 patients undergoing robotic-assisted thoracoscopic surgery were randomized to receive one of two injectate mixtures during their intraoperative INB.

There is a widely recognized need for nursing faculty in the United States. To prepare a practice-ready workforce, schools of nursing are hiring faculty with Doctor of Nursing Practice (DNP) preparation to ensure clinical expertise is embedded into curriculum by practice experts. However, nurses transitioning from clinical nursing to faculty positions require tailored support and guidance in navigating the academic environment.

Application of an Antiracism Framework to Improve Pediatric APRN Education.

Poor health outcomes for children are directly tied to poor social determinants of health and systemic, embedded health care inequities. To prepare the next generation of pediatric advanced practice registered nurses (APRNs), nursing schools must educate students to address inequities through innovative curricular models and teaching modalities. The purpose of this manuscript is to describe the application of an antiracism framework to a graduate APRN program.

Through the Looking Glass: Diversity, Equity, Inclusion, and Belonging: A Call for Action from the National Association of Pediatric Nurse Practitioners.

In response to growing health disparities, social inequities, structural racism, and discrimination, the National Association of Pediatric Nurse Practitioners established a Diversity, Equity, and Inclusion Taskforce. In 2020, this group transitioned into a national committee to infuse equity across the organization and empower pediatric-focused advanced practice registered nurses as agents of change to address health disparities. Emphasizing the critical need for understanding health disparities in the context of racism and discrimination, this committee champions a paradigm shift, transcending educational initiatives, advisory roles, advocacy efforts, leadership strategies, and community services to illuminate an equitable future for all children and families.

Evaluating a Health Equity Podcast for Provider Practice Change: A Cross-sectional Study.

Introduction: Podcasts are effective tools for disseminating health education. This study aimed to disseminate a health equity curriculum on best practices.

Method: A cross-sectional descriptive study was conducted as part of the Health Equity in Pediatrics podcast series between May and June 2023.

In machine learning, understanding happens through assimilated knowledge and is centered on six pillars: big data, data volume, value, variety, velocity, and veracity. This study analyzes school attendance problems (SAP), which encompasses its legal statutes, school codes, students' attendance behaviors, and interventions in a school environment.

Application of the Toxic Stress Schema: An Exemplar for School Nurses.

For many children of color, the cumulative impact of pre-existing stressors, disparities, and pandemic-related losses has contributed to a toxic level of stress. Toxic stress can disrupt healthy brain development making children vulnerable to physical and mental health problems. School nurses are the primary health professionals who interact with children daily during the academic year, which positions them to identify risk factors within the social determinants of health that contribute to the development of toxic stress.

Restoring trust in research among historically underrepresented communities: A call to action for antiracism research in nursing.

Background: Health disparities and inequities among historically underrepresented populations represent a public health crisis.

Purpose: This manuscript describes how use of an antiracism framework, an evidence-based approach to addressing systemic barriers, can assist with restoring trust in historically underrepresented communities that have experienced harm by researchers.

Methods: We also discuss the necessity of inclusive research teams and provide exemplars of how antiracism research principles and inclusive research teams can be used to mitigate harm and restore trust in historically underrepresented communities.

Minimally Invasive Treatment of Tunneled Central Venous Catheter-Associated Venobronchial Fistula.

Venobronchial fistula (VBF) is a rare complication of central venous access. We describe a 30-year-old woman with VBF associated with a tunneled venous catheter. She presented with a drowning sensation associated with infusions.

A Schema of Toxic Stress Informed by Racism, Transgenerational Stress, and Disadvantage.

Introduction: The Toxic Stress Schema (TSS) is an ecological framework with a social justice lens for identifying and alleviating stress and strengthening social determinants of health for children and families of color impacted by the COVID-19 pandemic and the cumulative effects of racism and generational, systemic inequities.

Method: Relevant literature is reviewed, and examples were provided to illustrate the differential impacts of the "stress superstorm" of 2020 had on children of color based on their family's position on the advantage-disadvantage continuum.

Results: The utility of the TSS framework as a model for advanced nursing practice is demonstrated, and recommendations are formulated for the pediatric nurse practitioner's role in health policy.

Effect of Mixing Method on Properties of Ethylene Vinyl Acetate Copolymer/Natural Rubber Thermoplastic Vulcanizates.

Thermoplastic vulcanizate (TPV) has excellent elastomeric properties and can be reprocessed multiple times. TPV is typically produced by using the dynamic vulcanization (DV) method in which rubber is crosslinked simultaneously with thermoplastics. Peroxide-crosslinked TPV can increase the compatibility between rubber and thermoplastics but loses its reprocessability due to excess crosslinking in the latter.

Spatial heterogeneity of radiolabeled choline positron emission tomography in tumors of patients with non-small cell lung cancer: first-in-patient evaluation of [F]fluoromethyl-(1,2-H)-choline.

: The spatio-molecular distribution of choline and its metabolites in tumors is highly heterogeneous. Due to regulation of choline metabolism by hypoxic transcriptional signaling and other survival factors, we envisage that detection of such heterogeneity in patient tumors could provide the basis for advanced localized therapy. However, non-invasive methods to assess this phenomenon in patients are limited.

Liposomal Bupivacaine Versus Bupivacaine for Intercostal Nerve Blocks in Thoracic Surgery: A Retrospective Analysis.

Background: Liposomal bupivacaine (LipoB), delivered via intercostal nerve blocks (ICNBs), is increasingly being used for postoperative pain control in thoracic surgery patients, but there is limited data on its effectiveness when compared to standard bupivacaine.

Objective: We sought to compare postoperative opioid use, pain control, and length of stay (LOS) in patients undergoing thoracic surgery with LipoB ICNBs vs patients undergoing thoracic surgery with ICNBs using standard bupivacaine.

Inferior vena cava filter placement for venous thromboembolism has increased by 25-fold in the past two decades. Timely retrieval of these filters is often not executed, resulting in long-term complications. We report a case of 44-year-old male patient who underwent inferior vena cava filter placement for chronic venous thromboembolism after presenting with hemoptysis.

Alternative approaches to environmental regulation have gained much attention in recent years. Information-based regulation is an increasingly popular type of instrument that refers to the use of ratings, rankings, labels, online inventories and similar public disclosure practices by regulators. Previous research regarding burnout in acute care settings revealed that positive work environments increase job satisfaction, decrease staff turnover, and improve patient outcomes. Asanguinous Del Nido Cardioplegia for an Aortic Valve Replacement Patient with Cold Agglutinins.

A patient with known cold agglutinins requiring an aortic valve replacement was referred for surgery. Asanguinous, Del Nido cardioplegia was used for myocardial protection. Warm induction followed by cold infusion prevented any agglutination and eliminated the need for subsequent cardioplegia doses.

Parental Knowledge about Urban Preschool Children’s Oral Health Risk.

Dental caries is one of the most prevalent and significant health problems in the United States. According to the American Academy of Pediatric Dentistry, more than one-fourth of children between the ages of two and five years experience early childhood caries before entering kindergarten. The purpose of this study was to explore parent’s knowledge of preschool children’s oral health risk factors.
