The authors report an unusual mode of onset of diffuse mesangial sclerosis in a newborn. Fatal neonatal renal failure, and not infantile nephrotic syndrome, was the main symptom after birth. The authors report a case of a tumour of the liver in a pregnant woman who had taken oral contraception for six years. Since oral contraceptives have become far more widely used the two benign tumours of the liver, namely a hepatocellular adenoma of the liver and focal nodular hyperplasia, have become far more common. The increased blood supply to the liver can make such a tumour symptomatic and increase the risk of it rupturing intraperitoneally, especially at the onset of labour.

At the conclusion of our recently published article in this Journal on low dose rate irradiation in moderately extensive cancers of the oropharynx, we updated our results in May 1986. Here we report on an expanded group of 65 patients with a 2 year minimum follow-up; 32 patients were treated by low dose rate irradiation and 33 by conventional fractionation. Forty-four percent (14/32) low dose irradiation patients survived with NED vs 8/33 (24%) conventional fractionation patients.

From 1977 to 1984, we treated 34 patients with mycosis fungoides and 9 patients with B cutaneous lymphomas. Eighteen patients with mycosis fungoides were treated with total skin electron irradiation (TSEI) and had a minimum follow-up of 15 months (range 15 months to 7 years). The lowest electron energy of the linear accelerator was 8 MeV therefore we placed a plexiglas screen between the patient and the machine; the resulting electron energy was 4 MeV.

The prognosis of high risk congenital diaphragmatic hernia and eventration diagnosed in the early neonatal period (before 24 h) is studied based on a series of 64 cases. Eventration has a poor prognosis with 5 deaths out of 7 cases. The replacement of the hemidiaphragm by an abdominal muscular flap seems to be the best surgical procedure (2 recoveries on 4 cases).

The authors report a case of respiratory distress secondary to inhalation of meconial amniotic fluid treated by mechanical ventilation, which was complicated by severe interstitial emphysema. Synapses develop concurrently and at identical rates in different layers of the visual, somatosensory, motor, and prefrontal areas of the primate cerebral cortex. This isochronic course of synaptogenesis in anatomically and functionally diverse regions indicates that the entire cerebral cortex develops as a whole and that the establishment of cell-to-cell communication in this structure may be orchestrated by a single genetic or humoral signal. This is in contrast to the traditional view of hierarchical development of the cortical regions and provides new insight into the maturation of cortical functions.

Between 1961 and 1975, 300 women with cancer of the breast (T1:72; T2:167; T3:61) were treated conservatively with radiotherapy alone (T3) or with partial or predominant radiotherapy combined with tumorectomy without axillary dissection (T1 or T2). The results, normalized at 10 years, confirmed that the survival rate (crude or NED) was exactly the same as with other conservative treatments or mutilating surgical treatments. Relapses were few (8%) in T1 cases and acceptable in T2 and T3 cases (13% and 18% respectively); in almost every patient they were amenable to surgery, with good local postoperative course.

Over a 6 years period, 51 autopsies have been performed in infants dead after severe sepsis. One third of them had renal damages, consisting mainly in haemorrhagic infiltration. Pathological and clinical data are not always well correlated, but some parameters may enhance a poorer renal prognosis, such as the duration of shock and anuria, or exchange transfusion.

The role of neuromuscular activity in the maturation of the motor innervation was investigated in the fast focally innervated posterior latissimus dorsi (PLD) muscle of the chick embryo. The axonal supply in the PLD motor nerve, and the focal multiple innervation of the endplates were described on days 15 and 16 of embryonic life in normal and experimental embryos. The few clinical studies which have utilized irradiation as a treatment modality for chronic lymphocytic leukaemia (CLL) during the last two decades have led to rather conflicting and sometimes disappointing results. Low-dose total body irradiation (TBI) and extracorporeal, or mediastinal, radiotherapy did not appear to be superior to chemotherapy in most trials. Only the fractionated low-dose (600-800 rad) splenic irradiation was found to induce a long-lasting decrease of the lymphocyte count and a decrease in bone marrow infiltration in a significant proportion of cases, without any noticeable haematological toxicity.
