Currently, there is no test system, whether in vitro or in vivo, capable of examining all endpoints required for genotoxicity evaluation used in pre-clinical drug safety assessment. The objective of this study was to develop a model which could assess all the required endpoints and possesses robust human metabolic activity, that could be used in a streamlined, animal-free manner. Objectives: This study aims to assess whether a simulation-based training programme focusing on palliative care emergencies conducted in a hospice setting could improve the self-reported confidence and competence of nursing staff.

Methods: A training programme was developed to enable nursing professionals to practice clinical skills necessary for recognising and managing palliative care emergencies including opioid induced respiratory depression, catastrophic haemorrhage, anaphylaxis, seizure and acute airway obstruction. Purpose: Neoadjuvant gemcitabine and cisplatin (GC) followed by radical cystectomy (RC) is standard for patients with muscle-invasive bladder cancer (MIBC). On the basis of the activity of atezolizumab (A) in metastatic BC, we tested neoadjuvant GC plus A for MIBC.

Methods: Eligible patients with MIBC (cT2-T4aN0M0) received a dose of A, followed 2 weeks later by GC plus A every 21 days for four cycles followed 3 weeks later by a dose of A before RC.

To accomplish the deceptively simple task of perceiving the size of objects in the visual scene, the visual system combines information about the retinal size of the object with several other cues, including perceived distance, relative size, and prior knowledge. When local component elements are perceptually grouped to form objects, the task is further complicated because a grouped object does not have a continuous contour from which retinal size can be estimated. Here, we investigate how the visual system solves this problem and makes it possible for observers to judge the size of perceptually grouped objects.

Background: The potential benefit of the current mycophenolic acid, mycophenolate mofetil, has not yet been fully achieved in clinical transplantation because of its gastrointestinal side effects. Introduction: The introduction of calcineurin inhibitors (CNIs) in clinical transplantation has resulted in dramatic reduction in acute rejection rate and improvements in short-term allograft survival. Studies using burst comparison procedures to examine age-related changes in intensity discrimination have reported that the ability to discriminate differences in intensity does not reach maturity until late childhood. In the present study, developmental changes in intensity discrimination were examined in 1- to 3-year-old children, using an increment detection paradigm. Children and adults detected increments in a continuous standard presented at three levels ranging from 35 to 55 dB SPL.

A visually reinforced operant paradigm was employed to examine the relationship between the difference limen (DL) for intensity and level of the standard during infancy. In Experiment 1, 7-month-old infants and adults detected increments in continuous noise presented via headphones at each of four levels ranging from 28 to 58 dB SPL. Noise stimuli were 2-octave bands centered at either 400 or 4000 Hz, and increments were 10 and 100 msec in duration.

The t(1;19)(q23;p13), detected cytogenetically in 5-6% of cases, is one of the most common translocations in childhood acute lymphoblastic leukemia (ALL). Most t(1;19)+ ALLs are pseudodiploid or contain fewer than 50 chromosomes, are classified as pre-B based on expression of cytoplasmic, but not surface, immunoglobulin (clg+/slg-), express a characteristic pattern of cell surface antigens, and contain E2A-PBX1 fusion mRNAs. A minority of cases are early pre-B (clg-/slg-), do not express the characteristic pattern of cell surface antigens, and lack E2A-PBX1 fusion mRNAs.

The possible benefits of premedication with the antispasmodic hyoscine n-butyl bromide (hyoscine) and analgesia with inhaled nitrous oxide/oxygen mixture (nitrous oxide) were assessed in a double-blinded, placebo-controlled trial. Consecutive patients at normal risk for cancer undergoing screening flexible sigmoidoscopy were randomly allocated to receive either (1) intravenous hyoscine 20 mg plus inhaled oxygen on demand (n = 40), (2) sterile water injection plus inhaled nitrous oxide on demand (n = 48), or (3) sterile water injection plus inhaled oxygen on demand (n = 43). One recently trained primary care physician performed all procedures.

A visually reinforced operant procedure was employed to obtain 2-point threshold-duration functions in 7-month-old infants and adults in two experimental paradigms. In Experiment 1, thresholds were determined for 10- and 100-msec, 500-Hz tones and octave-band noise bursts presented in quiet and noise backgrounds. Threshold-duration functions were significantly steeper for infants than for adults under all experimental conditions, and did not differ in slope as a result of differences in either stimulus bandwidth or masking noise.
