This guideline provides consensus opinion on the investigations required for people presenting with suspected monoclonal gammopathy of renal significance to both nephrology and haematology physicians. The guideline discusses the principles of treating a patient with MGRS and provides recommendations for both supportive management and haematological therapy. It details the recommended on-going monitoring required for both specialty areas.

Despite a varied selection of available trauma-focused evidence-based psychotherapies (TF-EBPs) for posttraumatic stress disorder (PTSD), few veterans receive a full course of an evidence-based treatment. A better understanding of and alignment with veterans' PTSD treatment goals could be one way to improve treatment engagement and adherence, consistent with veteran-oriented care within the U.S.

Article Synopsis
  • - Alcohol-related mortality rates in the U.S. are rising, particularly among women, due to factors like local alcohol policies and community socioeconomic conditions, suggesting that these broader social determinants impact health outcomes.
  • - A literature review conducted from March to May 2023 identified relevant studies from databases, focusing on area-level social determinants and their correlation with alcohol-related deaths while excluding non-peer-reviewed or irrelevant articles.
  • - Out of 313 abstracts screened, 71 studies were selected for in-depth analysis, highlighting the importance of understanding these social factors to develop effective health interventions.

Purpose Of Review: Research demonstrates a strong association between alcohol use disorder (AUD) and posttraumatic stress disorder (PTSD). However, less is known about sex- and gender-based differences among individuals with AUD + PTSD. This narrative review examines recent literature in this area and aims to be a reference for future research endeavors.

Empirically supported treatments for posttraumatic stress disorder (PTSD) exist, but research suggests these therapies are less effective, acceptable, and feasible to deliver to active duty service members (SMs) compared to civilians. Stellate ganglion block (SGB) procedure, in which a local anesthetic is injected around the cervical sympathetic chain or stellate ganglion to temporarily inhibit sympathetic nervous activity, is gaining popularity as an alternative PTSD treatment in military settings. However, it is unknown whether certain PTSD symptoms are more responsive to SGB than others.

Antagonist peptides (ANTs) of vasoactive intestinal polypeptide receptors (VIP-Rs) are shown to enhance T cell activation and proliferation in vitro, as well as improving T cell-dependent anti-tumor response in acute myeloid leukemia (AML) murine models. However, peptide therapeutics often suffer from poor metabolic stability and exhibit a short half-life/fast elimination in vivo. In this study, we describe efforts to enhance the drug properties of ANTs via chemical modifications.

Modern organic synthesis requires methodologies that bring together abundant feedstock chemicals in a mild and efficient manner. To aid in this effort, we have developed a multicomponent radical hydroxyarylation reaction that utilizes aryl halides, olefins, and O as the reaction components. Crucial to this advance was an oxidative, rather than a reductive, approach to aryl radical generation, which enables reaction tolerance to O.

Chiral aziridines are important structural motifs found in natural products and various target molecules. They serve as versatile building blocks for the synthesis of chiral amines. While advances in catalyst design have enabled robust methods for enantioselective aziridination of activated olefins, simple and abundant alkyl-substituted olefins pose a significant challenge.

Objective: Although mental health disorders and high-risk substance use frequently co-occur, they are typically investigated independently. Clinical trials focused on treatment for individuals with trauma- and stressor-related disorders often exclude individuals with high-risk substance use. Little is also known about the role of gender in the relationship between trauma- and stressor-related disorders and high-risk substance use.

Planar chirality is an important form of molecular chirality that can be utilized to induce enantioselectivity when incorporated into transition metal catalysts. However, due to synthetic constraints, the use of late transition metal planar chiral complexes to conduct enantioselective transformations has been limited. Additionally, the published methods surrounding the stereochemical assignment of planar chiral compounds are sometimes conflicting, making proper assignment difficult.

Background: Light chain proximal tubulopathy (LCPT) is a rare form of paraprotein-related disease, occurring in two main histopathological forms: crystalline and non-crystalline. The clinicopathological features, treatment strategies and outcomes, especially of the non-crystalline form, are not well described.

Methods: We conducted a single-centre retrospective case series of 12 LCPT patients, 5 crystalline and 7 non-crystalline, between 2005 and 2021.

Aims: Previous studies in adult congenital heart disease (CHD) have demonstrated a link between renal dysfunction and mortality. However, the prognostic significance of renal dysfunction in CHD and decompensated heart failure (HF) remains unclear. We sought to assess the association between renal dysfunction and outcomes in adults with CHD presenting to our centre with acute HF between 2010 and 2021.

Here, we report an efficient and modular approach toward the formation of difluorinated arylethylamines from simple aldehyde-derived -dialkylhydrazones and trifluoromethylarenes (CF-arenes). This method relies on selective C-F bond cleavage via reduction of the CF-arene. We show that a diverse set of CF-arenes and CF-heteroarenes react smoothly with a range of aryl and alkyl hydrazones.

Article Synopsis
  • The study challenges the assumption that all PTSD symptoms are equally important for diagnosing the disorder, finding that some symptoms carry more weight than others.
  • The research analyzed data from 2,658 individuals, using both traditional clinical interviews and new statistical methods to establish a more accurate PTSD diagnosis.
  • Results showed that 25% of patients had a PTSD diagnosis that didn't match their actual symptom severity, particularly affecting veterans, indicating that improved diagnostic standards could help better identify and treat PTSD.

Objectives: A seminal study on loneliness and depression suggested that loneliness influences depression more than the reverse. However, the study's analytic method has since been criticized for failing to account for the trait-like nature of variables. This study aimed to examine the longitudinal relationship between loneliness and depressive symptoms while accounting for the trait-like nature of both variables.

Article Synopsis
  • * Key findings revealed that younger age, substance use disorders, and being male significantly increased the likelihood of legal trouble post-discharge, especially for veterans from lower socioeconomic neighborhoods.
  • * The research highlights the importance of addressing mental health and substance use issues among veterans, as those with multiple disorders faced a tenfold higher risk of legal involvement compared to those without.

Aim: Triage is key to effective management of major incidents, yet there is scarce evidence surrounding the optimal method of paediatric major incident triage (MIT). This study aimed to derive consensus on key components of paediatric MIT among healthcare professionals responsible for triage during paediatric major incidents.

Methods: Two-round online Delphi consensus study delivered July 2021-October 2021, including participants from pre-hospital and hospital specialities responsible for triage during paediatric major incidents.

Ribosomally synthesized and post-translationally modified peptides (RiPPs) are known for their macrocyclic structures, which impart unique biological activity. One rapidly emerging subclass of RiPP natural products contains macrocyclic C-C cross-links between two amino acid side chains. These linkages, often biosynthetically formed by a single rSAM or P450 enzyme, introduce significant structural and synthetic complexity to the molecules.

Complementary and integrative health (CIH) approaches have gained empirical support and are increasingly being utilized among veterans to treat a myriad of conditions. A cluster of medically unexplained chronic symptoms including fatigue, headaches, joint pain, indigestion, insomnia, dizziness, respiratory disorders, and memory problems, often referred to as Gulf War Illness (GWI) prominently affect US Gulf War era (GWE) veterans, yet little is known about CIH use within this population. Using data collected as part of a larger study (n = 1153), we examined the influence of demographic characteristics, military experiences, and symptom severity on CIH utilization, and utilization differences between GWE veterans with and without GWI.
