A central surveillance register for all bacillary pulmonary tuberculosis cases reported in Bavaria (population in excess of 10 million) was established from 1974 to 1976. The aim of the study was to discover the quality and efficiency of health services delivery to the population in the field of tuberculosis under routine conditions, and to find out the relapse rate after cessation of chemotherapy in expatients who were found to be negative 2 years after starting chemotherapy. Nuclear medical imaging of the lung after inhalation of Tc-labelled aerosols can be used instead of ventilation scintigraphy with Xe-133 in those cases, where the single-breath-period is essential for diagnosis. Especially for pulmonary embolism aerosols have some advantages compared with Xe-133: Scintigraphic images can be done in several views, aerosols are always available, there is no problem with room-contamination. In patients with obstructive lung disease or emphysema, where the wash-out-period is more essential for diagnosis than the single-breath-period, there is an advantage of xenon to aerosols.

Fifty-five peripheral pulmonary lesions in 46 patients were examined by computed tomography. The results were correlated with histologic diagnoses. The series comprised 26 bronchial carcinomas, 17 metastases, 4 chondromas, 5 tuberculomas and 3 echinacoccal cysts.

Eight cases of endothoracal carcinoma associated with pulmonary fibrosis or with interstitial pneumonia are reported. Using the farmer's lung as an example, the various problems in establishing a diagnosis are discussed. The "instruments" of diagnosis such as history (exposition), functional parameters, macro- and micromorphology, and laboratory findings are described. The different criteria and their value, the compatibility of informations, and the quality of definitions ("fuzzy sets") are analytically evaluated--all this finally leading to the "quality" of diagnosis and of subsequent decisions.
