Background: NHS England issued commissioning guidance on 18 low-priority treatments which should not be routinely prescribed in primary care. We aimed to monitor the impact of an educational intervention delivered to regional prescribing advisors by senior pharmacists from NHS England on the primary care spend on low-priority items.

Methods: An opportunistic randomised, controlled parallel-group trial.

Article Synopsis
  • The study investigates the connection between thrombo-inflammatory and oxidative stress biomarkers and erythropoiesis-stimulating agent (ESA) hyporesponsiveness in chronic hemodialysis patients.
  • It included 96 patients, analyzing various biomarkers and clinical data to calculate the ESA-hyporesponsiveness index (EHRI).
  • Findings indicate that low body mass index (BMI) and high levels of L-type fatty acid binding protein (L-FABP) are significant predictors of ESA resistance, suggesting these factors could help identify patients at risk for reduced responsiveness to ESA therapy.

Article Synopsis
  • - The study investigated cardiac changes in 40 patients with rheumatic mitral stenosis (MS) using cardiovascular magnetic resonance, finding significant structural and functional differences compared to a control group.
  • - Key findings included lower ejection fractions for both the left and right ventricles, increased volumes for the right ventricle and left atrium, and a high prevalence of late gadolinium enhancement (LGE) suggesting tissue damage.
  • - The results highlighted that biventricular dysfunction and atrial remodeling are significant yet often overlooked features in patients with rheumatic MS, indicating a need for further research into their long-term implications.

Article Synopsis
  • The study aimed to evaluate how well Apparent Diffusion Coefficient (ADC) values can distinguish between benign and malignant skull lesions compared to conventional MRI techniques.
  • A total of 53 patients underwent both conventional MRI and Diffusion-Weighted Imaging (DWI), with statistical analysis comparing ADC values of lesions against histopathological findings.
  • Results indicated that incorporating ADC values improved diagnostic accuracy for differentiating lesions, achieving 75% accuracy, with ADC proving to be a significant non-invasive tool in clinical assessments.

Objectives: Cor Triatriatum is a rare anomaly that can either involves the left atrium (Cor Triatriatum Sinister-CTS) or the right atrium (Cor Triatriatum Dexter- CTD). Preoperative identification of this anomaly is important in determining patient treatment course. The objective of this paper is to understand imaging findings, classification and to familiarise the reader with other associated congenital cardiac anomalies that influence patient management.

Although MRI has a spectrum of findings which help in the diagnosis of tuberculosis (TB) spine, a broad spectrum of spine pathologies resemble Pott's spine on MRI and are often missed due to inadequate clinical details. As a result, patients are often subject to unnecessary biopsy. A blinded radiologist may misdiagnose such mimic cases as TB.

Dorsal dermal sinus (DDS) is an uncommon type of occult spinal dysraphism most often located in the lumbar region. Patients present either due to secondary infection or compression of neural structures by an associated dermoid or epidermoid cyst. We report a rare case of 2-year-old child who presented with progressive paraparesis with magnetic resonance imaging of spine showing a thoracic DDS with an infected intramedullary dermoid cyst.
