This proposed scientific statement is focused on providing new insights regarding challenges and opportunities for cardiovascular health (CVH) promotion in Africa. The statement includes an overview of the current state of CVH in Africa, with a particular interest in the cardiometabolic risk factors and their evaluation through metrics. The statement also explains the main principles of primordial prevention, its relevance in reducing noncommunicable disease and the different strategies that have been effective worldwide.

Article Synopsis
  • A study was conducted to assess the prevalence of hospital-acquired infections in patients who stayed in the hospital for at least 48 hours, revealing a 12.3% infection rate among the 463 patients studied.
  • The majority of infections were urinary, surgical site, and bacteremia, with a notable presence of invasive devices in most cases, and a diverse range of germs identified, including enterobacteria and resistant strains.
  • The findings highlight the significant occurrence of nosocomial infections and the importance of further research to better understand and address this issue.

Anticoagulant-induced spontaneous small bowel hematoma is a rare cause of acute mechanical bowel obstruction. We report the case of a 77-year-old patient with complete arrhythmia due to atrial fibrillation under acenocoumarol 4mg daily who was seen for right iliac fossa pain. The laboratory tests showed a prothrombin rate (PT) of 12%, an International Normalized Ratio (INR) of 6, and an aPTT (activated partial thromboplastin time) of 43 seconds.

Article Synopsis
  • The study presents a new ultrasound-guided technique for blocking the posterior femoral cutaneous nerve (PFCN) called the "Gluteus-Deep Investing Fascia compartment Block" (GDIF block), which targets the space between the gluteus maximus muscle and deep investing fascia.
  • Nineteen above-knee surgeries were performed on nine patients using the GDIF block as part of the Complete Lower Extremity Fascia Tri-compartment Block (CLEFT Block) without complications.
  • Results indicate that the GDIF block effectively achieved complete PFCN blockade, allowing surgeries to proceed without the need for additional pain relief or conversion to general anesthesia.

  • * Seven patients were treated using this nerve block method in district hospitals between May and December 2023, successfully completing their surgeries without complications or the need for general anesthesia.
  • * The findings suggest that this combined technique is a safe option for high-risk surgical patients, although further research with a larger sample size is needed to confirm effectiveness.

Article Synopsis
  • Lung ultrasound is an effective non-invasive tool for assessing lung injury in critically ill COVID-19 patients, especially in low-income settings like Mali.
  • A study involving 156 patients showed that most had respiratory failure, with high feasibility and reproducibility in the ultrasound assessments.
  • Findings revealed a significant correlation between lung ultrasound scores and oxygen levels, with higher scores linked to increased mortality, emphasizing the method's value in critical care decisions.

Introduction: atrial fibrillation (AF) is the most common cardiac rhythm disorder. Its prevalence is underestimated in Africa, hence the initiation of the Atrial Fibrillation Registry In Countries of Africa (AFRICA). The aim of our study was to describe, within the framework of the AFRICA registry, the epidemiological, clinical, paraclinical, therapeutic and evolutionary aspects of atrial fibrillation (AF) in Africa, particularly in Senegal.

Point-of-care ultrasound (POCUS) is an increasingly accessible skill, allowing for the decentralization of its use to nonspecialist healthcare workers to guide routine clinical decision-making. The advent of ultrasound-on-a-chip has transformed the technology into a portable mobile health device. Because of its high sensitivity to detect small consolidations, pleural effusions, and subpleural nodules, POCUS has recently been proposed as a sputum-free likely triage tool for tuberculosis (TB).

Introduction: Over the past two decades, the incidence of acute rheumatic fever (ARF) and chronic rheumatic heart disease (RHD) have dramatically declined in wealthier regions of the world as a result of preventative programmes, improved living standards and access to cardiac surgery. Nevertheless, ARF and RHD are still public health problems in less-developed regions of the world such as Oceania, south Asia and sub-Saharan Africa.

Aim: We report on clinical, therapeutic and prognostic aspects as well as the difficulties encountered during this first series of surgery for rheumatic valve disease in Mali.

Mitral valve repair is a better therapeutic alternative than valve replacement for rheumatic valve disease in children. Repair procedures are especially well suited to developing countries where heart prostheses and life-long anti-coagulation therapy are largely unaffordable. The purpose of this study was to evaluate medium-term outcome of mitral valve repair in children in Senegal.
