Background: Performing a left atrial appendage occlusion (LAAO) or catheter ablation with left-sided intracardiac thrombus is considered very-high risk for periinterventional stroke. Cerebral embolic protection (CEP) devices are designed to prevent cardioembolic stroke and have been widely studied in TAVR procedures. However, their role in LAAO and catheter ablation of ventricular tachycardia (VT) or in pulmonary vein isolation (PVI) with cardiac thrombus present remains unknown.

Assessing physicians' well-being, occupational demands, and resources can help create more supportive and health-promoting working environments.

Most of our knowledge of deep sedimentary life comes from marine environments; however, despite their relatively small volume, lacustrine sediments constitute one of the largest global carbon sinks and their deep sediments are largely unexplored. Here, we reconstruct the microbial functional and taxonomic composition of an 8,000-year Holocene sedimentary succession from meromictic Lake Cadagno (Switzerland) using shotgun metagenomics and 16S rRNA gene amplicon sequencing. While younger sediments (<1,000 years) are dominated by typical anaerobic surface sedimentary bacterial taxa (, and , older layers with lower organic matter concentrations and reduced terminal electron acceptor availability are dominated by taxa previously identified as "persistent populations" within deep anoxic marine sediments ( Bathyarchaeia, , and ).

Background And Objective: The reverse total shoulder arthroplasty (RSA) is a widely used innovative procedure for managing shoulder pathologies like severe rotator cuff arthropathy, osteoarthritis with significant glenoid deformity, or proximal humerus fractures. RSA prosthesis designs utilize the deltoid muscle to bypass the role of the rotator cuff, and to generate most of the force required for shoulder elevation. As such, preoperative deltoid insufficiency or injury, as well as any intraoperative or postoperative complications involving the deltoid, can significantly impact patient outcomes, rehabilitation, and recovery following RSA.

It is widely accepted that cancer mostly arises from random spontaneous mutations triggered by environmental factors. Our theory challenges the idea of the random somatic mutation theory (SMT). The SMT does not fit well with Charles Darwin's theory of evolution in that the same relatively few mutations would occur so frequently and that these mutations would lead to death rather than survival of the fittest.

Knee muscle strength and movement biomechanics in individuals with and without knee pain after anterior cruciate ligament reconstruction: A cross-sectional study.

Purpose: Anterior cruciate ligament injury increases the risk of knee osteoarthritis, possibly via early onset of knee pain and changes in musculoskeletal function. This study compared knee muscle strength and movement biomechanics during walking and forward lunge between individuals with and without knee pain after anterior cruciate ligament reconstruction.

Methods: Cross-sectional study including participants at least 3 years post anterior cruciate ligament reconstruction, aged 18-40 at the time of surgery, and body mass index ≤30.

To facilitate effective preclinical testing of senescence treatments for osteoarthritis (OA), we have created reliable biomimetic and high-throughput models using aged human joint tissues. Moreover, concerns regarding scalability led to the concurrent development of a high-throughput human in vitro senescence cartilage organoid model. Osteochondral explants and cells for the cartilage organoid model were isolated from patients undergoing joint replacement surgery due to OA.

Background: Lumbosacral transitional vertebra (LTV) is a congenital anomaly in dogs and have been proposed to be associated with cauda equina syndrome (CES) in German Shepherd dogs. This prospective study, including 32 dogs from two breeds, aims to investigate if LTV is associated with lower back pain in dogs. The study compared owners' assessment of their dogs' well-being and clinical evaluation with advanced diagnostic imaging to identify changes that might explain clinical findings.

Decoupling microbial iron reduction from anoxic microsite formation in oxic sediments: a microscale investigation through microfluidic models.

Iron (Fe) reduction is one of the oldest microbial processes on Earth. After the atmosphere and ocean became oxygenated, this anaerobic process was relegated to niche anoxic environments. However, evidence of Fe reduction in oxic, partially saturated subsurface systems, such as soils and vadose zones, has been reported, with the common explanation being the formation of anoxic microsites that remain undetected by bulk measurements.

Low-dimensional materials with charge density waves (CDW) are attractive for their potential to exhibit superconductivity and nontrivial topological electronic features. Here we report the two-dimensional (2D) chalcogenide, BaSbTeS which acts as a new platform hosting these phenomena. The crystal structure of BaSbTeS is composed of alternating atomically thin Te square-net layers and double rock-salt type [(SbTeS)] slabs separated with Ba atoms.

Shining a light on cell biology of the nucleus with single-cell sequencing.

From the preservation of genomic integrity to the regulation of RNA translation, nearly all cellular processes are regulated in a cell context-dependent manner. To fully understand the context-specific function of involved nuclear processes, a vast number of single-cell sequencing technologies were developed over the last decade. This instrumental work demonstrated the heterogeneity between cell types and individual cells, bringing about new understanding of nuclear mechanisms and their crosstalk to cell states.

Background: Baseline systemic inflammation is associated with worse long-term outcomes after coronary artery bypass grafting [CABG], but the mechanisms of this association are unclear. This study aims to explore the association between pre-operative white blood cell [WBC] count and CABG graft failure.

Methods: We pooled individual patient data from two randomized clinical trials with systematic CABG graft imaging.

Delirium is a highly prevalent neuropsychiatric syndrome characterised by acute and fluctuating impairments in attention and cognition. Mechanisms driving delirium are poorly understood but it has been suggested that blood cytokines and chemokines cross the blood brain barrier during delirium, directly impairing brain function. It is not known whether these molecules reach higher brain levels when the blood cerebrospinal fluid barrier (BCSFB) is impaired.

Large-scale evaluation of outcomes after a genetic diagnosis in children with severe developmental disorders.

Purpose: We sought to evaluate outcomes for clinical management after a genetic diagnosis from the Deciphering Developmental Disorders study.

Methods: Individuals in the Deciphering Developmental Disorders study who had a pathogenic/likely pathogenic genotype in the DECIPHER database were selected for inclusion ( = 5010). Clinical notes from regional clinical genetics services notes were reviewed to assess predefined clinical outcomes relating to interventions, prenatal choices, and information provision.

Background: A lumbosacral transitional vertebra (LTV) is a congenital anomaly of the caudal vertebral column. It has been associated with asymmetrical canine hip dysplasia (CHD) and cauda equina syndrome (CES) in German Shepherd dogs. This retrospective cross-sectional study aims to report the potential influence of asymmetric LTV on pelvic anatomy using ventrodorsal (VD) radiographs.

Design and implementation of an action plan for justice, equity, diversity, and inclusion within the Clinical Genome Resource.

How might members of a large, multi-institutional research and resource consortium foster justice, equity, diversity, and inclusion as central to its mission, goals, governance, and culture? These four principles, often referred to as JEDI, can be aspirational-but to be operationalized, they must be supported by concrete actions, investments, and a persistent long-term commitment to the principles themselves, which often requires self-reflection and course correction. We present here the iterative design process implemented across the Clinical Genome Resource (ClinGen) that led to the development of an action plan to operationalize JEDI principles across three major domains, with specific deliverables and commitments dedicated to each. Active involvement of consortium leadership, buy-in from its members at all levels, and support from NIH program staff at pivotal stages were essential to the success of this effort.
