Background: Wire-guided localization (WGL) for non-palpable breast cancer lesions has drawbacks like wire migration, localization difficulties, and logistical challenges. Wireless methods, such as reflector-guided localization (RGL), address these issues and are compatible with breast MRI. This study evaluates the organizational and cost impacts of RGL compared to WGL.

Article Synopsis
  • A study examined the prevalence of residual adenocarcinoma in situ (AIS) after hysterectomy in women who had clear margins from initial excisional therapy between 2008 and 2021.
  • Out of 53 patients, 20.8% showed residual disease, mainly consisting of persistent AIS, with no cases of invasive cancer.
  • The findings suggest that even with negative margins, a hysterectomy is recommended post-childbearing, as there’s a notable risk of residual disease.

  • Malnutrition is a big problem for people with ovarian cancer and can make their situation worse, so the study aimed to find the best way to check for it.
  • The researchers looked at data from 615 patients diagnosed between 2003 and 2020 and used different methods to assess their nutrition, like checking for weight loss and blood protein levels.
  • Although they didn’t find a clear way to define malnutrition that would help predict health issues or survival, they emphasized that monitoring nutrition is important for improving patients' outcomes.

  • Early management of high-grade cervical intraepithelial neoplasia is vital for cervical cancer prevention, with both loop electro excision (LEEP) and laser excision methods being effective techniques recommended for lesion removal.
  • The study aimed to compare these two methods, focusing on the quality of surgical margins, and was conducted on 231 patients over an 8-year period.
  • Results showed no significant difference in negative margin rates between the two techniques, but LEEP was associated with a shorter cone depth in excisions performed.

Introduction: Indocyanine green (ICG) for axillary sentinel lymph node (SLN) detection in early breast cancer is at least equivalent to radioisotopic and/or colorimetric techniques. This procedure has not yet been recommended by learned societies in France. Following the recent drug approval of Infracyanin ® in this indication, we wished to study the use of ICG in France.

Migraine represents the most common neurologic disorder, ranking second among the world's causes of disability [expressed as years lived with disability (YLDs)]. Patients often do not receive the best therapy because of safety issues, tolerance, and prescription accessibility. General practitioners are not always educated about the disease, and specialists are few and often difficult to reach.

Objective: To describe the patterns of recurrence and the prognosis of patients with a recurrent TP53 mutated endometrial carcinoma treated initially by surgery.

Methods: All patients with endometrial carcinoma, treated at hospital European Georges Pompidou between 2001 and 2021 were retrospectively included. Patients were separated into two groups: TP53-mutated and not TP53-mutated (POLE/ultramutated-like (POLEmut), dMMR (mismatch repair-deficient) and NSMP (No specific molecular profile)).

Article Synopsis
  • - The study compares survival and morbidity rates between two surgical approaches for advanced epithelial ovarian cancer: primary cytoreductive surgery (pCRS) and interval cytoreductive surgery (iCRS), using a matched cohort from the FRANCOGYN data.
  • - Analyzing 513 patients, the results showed no significant differences in overall survival (OS) or recurrence-free survival (RFS) between the two groups, even though the pCRS group experienced higher postoperative complications.
  • - Conclusions indicate that while there is no difference in survival rates, iCRS is associated with lower postoperative morbidity compared to pCRS, suggesting it may be a preferable surgical option.

Article Synopsis
  • This study looks at patients with early-stage ovarian cancer and whether they need to have a hysterectomy (removal of the uterus) or if they can keep it to have children later.
  • Out of 85 patients studied, only 6 had cancer that affected the uterus (which is about 7%), and there weren't many differences between those patients and those who didn't.
  • The research suggests that because there are very few patients involved, it's hard to make strong conclusions, but it might be possible to consider keeping the uterus for some patients during surgery.

Migraine is one of the most widespread and burdensome diseases, affecting one in every seven individuals in the world, for an estimated global prevalence of 14%. Until recently, therapeutic choices for older migraineurs have been limited by safety concerns and such patients have typically been excluded from clinical trials. However, randomized controlled trials (RCTs) of new migraine drugs have begun to include participants aged over 65 years, offering clinicians relevant safety and efficacy data to be able to treat older patients with the newest drug classes, including monoclonal antibodies for CGRP (r), CGRP antagonists, and drugs targeting the serotonin 5-HT receptor.

International Federation of Gynecology and Obstetrics (FIGO) staging classification for stage IV epithelial ovarian cancer (EOC) separates stages IVA (pleural effusion) and IVB (parenchymal and/or extra-abdominal lymph node metastases). We aimed to evaluate its prognostic impact and to compare survival according to the initial metastatic location. We conducted a multicenter study between 2000 and 2020, including patients with a FIGO stage IV EOC.

Migraine represents the most common cause of work disability in young women and the second one in the general population. Preventive treatment can reduce the frequency of attacks and their intensity, consequently improving the quality of life. Despite this, global health systems have shown important gaps in addressing optimal management of preventive therapy.

Neck disability and pain are frequently encountered problems in patients with chronic migraine (CM). The long-term stimuli of neurons in the trigeminocervical junction may explain this situation. OnabotulinumtoxinA (ONA) treatment is one of the proven treatments for CM; however, there is no study data on the efficacy of ONA treatment on neck disability and pain in CM patients.

The use of portable ultrasound (US) devices is increasing, due to its accessibility, versatility, non-invasiveness, and its significant support in the patient management, extending the traditional physical examination through the POCUS (point-of-care ultrasound). The pocket-size or handheld ultrasound devices (HUDs) can easily perform focused exams, not aiming to substitute for the high-end US systems (gold standard), since the HUDs usually have more limited functions. The HUDs are promising tools for the diagnosis, prognosis, and monitoring of the COVID-19 infection and its related disorders.

Headache is among the most frequent symptoms persisting or newly developing after coronavirus disease 2019 (COVID-19) as part of the so-called long COVID syndrome. The knowledge on long COVID headache is still limited, however growing evidence is defining the features of this novel condition, in particular regarding clinical characteristics, some pathophysiological mechanisms and first treatment recommendations. Long COVID headache can present in the form of worsening of a preexisting primary headache, or, more specifically, in the form of a new (intermittent or daily) headache starting during the acute infection or after a delay.

OVARIAN CANCER SURGERY Surgery has a central role in the management of patients with epithelial ovarian cancer, and the quality of its implementation is a factor influencing the prognosis. For advanced stages, it is schematically based on two main steps: peritoneal cytoreductive surgery, and pelvic and para-aortic lymph node dissections. While the prognostic impact of reducing the peritoneal tumor burden has been well demonstrated (with the objective of no macroscopic residue at the end of the procedure), the prognostic impact of lymph node surgery is still debated in certain indications.

The introduction of new drug classes for migraine, such as monoclonal antibodies that target the calcitonin gene-related peptide (CGRP) or its receptor and small-molecule antagonists of CGRP, have opened a new scenario in a large population of individuals suffering from migraines. The provision of an effective and safe therapy can help overcome the high social and personal costs together with the burden of this disease by offering social, work and economic recovery to the people affected by migraine. Whether the satisfaction of personal and collective unmet needs will be achieved in the vast majority of migraine sufferers now depends only on the efficiency of the organizational care structures dedicated to this socially impactful disease.
