Development and Evaluation of an Augmented Reality Ultrasound Guidance System for Spinal Anesthesia: Preliminary Results.

Applications of ultrasound guidance for epidural injections are hindered by poor needle and epidural space visualization. This work presents an augmented reality (AR) ultrasound guidance system that addresses challenges in both needle visualization during navigation and epidural space identification for needle positioning. In this system, (i) B-mode ultrasound and the needle are visualized in a 3-D AR environment for improved navigation, and (ii) A-mode ultrasound, obtained from a custom-made single-element transducer housed at the needle tip, is used to identify the epidural space for improved needle positioning.

The chromosome organization activities of SMC complexes are crucial for correct gene expression and genetic inheritance in cells. Hi-C assays have revealed previously unsuspected levels of chromosome structure, with different types of chromosome structure facilitating function at different stages of the cell cycle. Elucidating how SMC complexes regulate these distinct types of organization is currently a key question in molecular biology.

Plasmonic colours predicted by deep learning.

Picosecond laser pulses have been used as a surface colouring technique for noble metals, where the colours result from plasmonic resonances in the metallic nanoparticles created and redeposited on the surface by ablation and deposition processes. This technology provides two datasets which we use to train artificial neural networks, data from the experiment itself (laser parameters vs. colours) and data from the corresponding numerical simulations (geometric parameters vs.

Perspective: L-arginine and L-citrulline Supplementation in Pregnancy: A Potential Strategy to Improve Birth Outcomes in Low-Resource Settings.

The available data support the hypothesis that L-arginine or L-citrulline supplementation would be suitable for implementation in resource-constrained settings and will enhance placental vascular development and improve birth outcomes. In resource-constrained settings, the rates of adverse birth outcomes, including fetal growth restriction, preterm birth, and low birth weight, are disproportionately high. Complications resulting from preterm birth are now the leading cause of mortality in children <5 y of age worldwide.

Gastrocnemius fascicles are shorter and more pennate throughout the first month following acute Achilles tendon rupture.

The purpose of this study was to characterize the short-term effects of Achilles tendon ruptures on medial gastrocnemius. We hypothesized that the fascicles of the medial gastrocnemius muscle of the injured Achilles tendon would be shorter and more pennate immediately following the injury and would persist throughout 4 weeks post-injury. B-mode longitudinal ultrasound images of the medial gastrocnemius were acquired in 10 adults who suffered acute Achilles tendon ruptures and were treated non-operatively.

Background: The role of migration pathway (refugees vs. asylum seekers) is seldom addressed in extant literature that looks at gender and mental health of humanitarian migrants. The aim of this study is to assess the relationship between gender and psychological distress among humanitarian migrants in Australia including the potential moderating role of migration pathway.

Measuring clinically relevant knee motion with a self-calibrated wearable sensor.

Low-cost sensors provide a unique opportunity to continuously monitor patient progress during rehabilitation; however, these sensors have yet to demonstrate the fidelity and lack the calibration paradigms necessary to be viable tools for clinical research. The purpose of this study was to validate a low-cost wearable sensor that accurately measured peak knee extension during clinical exercises and needed no additional equipment for calibration. Sagittal plane knee motion was quantified using a 9-axis motion sensor and directly compared to motion capture data.

Mindfulness-based interventions for cancer-related pain and depression: a narrative review of current evidence and future potential.

Purpose Of Review: People with cancer commonly experience persistent pain and psychological distress. Interventions are needed which address the multifactorial nature of pain and depression, yet few studies have examined the impact of mindfulness-based interventions (MBIs) for cancer-related pain and depression.

Recent Findings: MBIs for cancer-related pain and depression can be effectively delivered across a range of modalities and show promise for alleviating mood and some physical health symptoms, although not always pain.

Prostate cancer detection using residual networks.

Purpose: To automatically identify regions where prostate cancer is suspected on multi-parametric magnetic resonance images (mp-MRI).

Methods: A residual network was implemented based on segmentations from an expert radiologist on T2-weighted, apparent diffusion coefficient map, and high b-value diffusion-weighted images. Mp-MRIs from 346 patients were used in this study.

Birth cohort, ageing and gender ideology: Lessons from British panel data.

Individuals' gender attitudes influence their behaviors, and adherence to traditional gender ideology is an important mechanism contributing to the (re)production of gender inequalities. In developed nations, the 'gender revolution' was accompanied by marked societal shifts towards gender-egalitarian attitudes, but these trends have recently stalled. In this paper, we re-examine the role of birth cohort and ageing in influencing gender ideology through the lens of life-course theory and leveraging British panel data.

The convergence of two DNA replication forks creates unique problems during DNA replication termination. In E. coli and SV40, the release of torsional strain by type II topoisomerases is critical for converging replisomes to complete DNA synthesis, but the pathways that mediate fork convergence in eukaryotes are unknown.

Large-area electrode design for vertical electrical impedance scanning of concrete bridge decks.

Vertical electrical impedance (VEI) testing is a method that was developed specifically for quantifying the level of protection against chloride ingress in concrete bridge decks. A significant operational difficulty associated with VEI testing has been establishing the required rebar tap as a ground reference. As described in this work, the use of a large-area electrode (LAE) can potentially remove the need for a rebar tap and therefore significantly simplify and accelerate the VEI testing process.

Parameterization of proximal humerus locking plate impingement with in vitro, in silico, and in vivo techniques.

Background: Locked plating of displaced proximal humeral fractures is common, but rates of subacromial impingement remain high. This study used a multidisciplinary approach to elucidate the relationships between common surgical parameters, anatomic variability, and the likelihood of plate impingement.

Methods: The experiment was completed in 3 phases.

Universal cervical length screening with a cervicometer to prevent preterm birth <34 weeks: a decision and economic analysis.

Preterm birth is a leading cause of neonatal morbidity and mortality worldwide; evidence-based strategies to decrease preterm birth are desperately needed. The purpose of this study was to estimate which of three strategies for screening for shortened cervix in asymptomatic low-risk women is the most cost-effective in terms of prevention of preterm birth and associated morbidity. A decision analysis model was developed from available published evidence comparing three strategies in screening asymptomatic low-risk women for shortened cervix: (1) cervicometer with subsequent referral for transvaginal ultrasound, (2) transvaginal ultrasound screening, and (3) no screening.

The scars of the past? Childhood health and health differentials in later life.

This study estimates multilevel mixed effects models of three retrospective measures of childhood health - self-rated childhood health, exposure to parental smoking growing up, and missing school for 30 or more consecutive days due to a health event - on levels and changes in physical functioning at age 50 and beyond. Using data from 15 waves of the Household, Income and Labour Dynamics in Australia survey, the results show that variation in the level of later-life physical functioning is associated with childhood health. Poor childhood health however is not associated with the rate of physical functioning decline.

However, the causal effects of muscle-tendon structure on joint function have not been tested in a controlled setting. Therefore, the purpose of this study was to test the implications of muscle-tendon unit parameters on simulated single-leg heel raise height.

Obstetric management of women with opioid use disorder.

Pregnancy presents a window of opportunity for effecting positive change in the lives of women with opioid use disorder (OUD). Care should be empathetic and nonjudgmental with a focus on counseling for initiation and maintenance of beneficial health behaviors as well as development of a strong patient-provider relationship. These include adherence to treatment of OUD through pharmacotherapy and behavioral counseling, smoking cessation, healthy nutrition, treatment of coexisting medical and psychosocial conditions, as well as preparation for the postpartum period through breastfeeding education and antenatal discussion of contraception.

A retrospective cohort study of fetal assessment following preterm premature rupture of membranes.

Objective: To evaluate maternal and neonatal outcomes following management of preterm premature rupture of membranes (PPROM) by two fetal assessment strategies.

Methods: In a retrospective cohort study performed at two hospitals in Philadelphia, Pennsylvania between July 2010 and June 2015, data were reviewed from 180 singleton pregnancies with PPROM at 23 -33  weeks of gestation that underwent expectant management. Outcomes were compared between continuous electronic fetal heart monitoring (EFM) with daily biophysical profile (BPP) ("continuous monitoring") and non-stress test (NST) three times per day ("periodic monitoring") using Mann-Whitney U and Fisher exact tests.

Utilizing a Faculty Development Program to Promote Safer Opioid Prescribing for Chronic Pain in Internal Medicine Resident Practices.

Objective: To implement a skills-based faculty development program (FDP) to improve Internal Medicine faculty's clinical skills and resident teaching about safe opioid prescribing.

Design: An FDP for Internal Medicine attendings that included a one-hour didactic presentation followed immediately by an Objective Structured Clinical Examination (OSCE) that focused on assessing and managing opioid misuse risk, opioid treatment outcomes (benefits and harms), and aberrant opioid use behaviors. The evaluation compared pre- and three-months-post-FDP changes in faculty's safe opioid prescribing knowledge, attitudes, confidence (clinical and teaching), and self-reported resident teaching.

Evaluation of additive effect of quantitative fetal fibronectin to cervical length for prediction of spontaneous preterm birth among asymptomatic high-risk women.

To determine if quantitative fetal fibronectin (qFFN) in addition to transvaginal ultrasound (TVU) cervical length (CL) measurement between 18 and 23 weeks would be predictive of spontaneous preterm birth (sPTB) at <35 weeks among asymptomatic high-risk women as defined by those with prior sPTB > 20 weeks. A prospective observational study of asymptomatic women with singleton gestations between 18 and 23weeks and one or more prior SPTB was performed. Women at their anatomy scan who opted into universal CL screening were enrolled.

Transvaginal ultrasound training for the obstetrics and gynecology resident: A multisite randomized controlled trial of educational DVD.

Purpose: Residents in obstetrics and gynecology are deemed to be proficient in transvaginal ultrasound (TVUS) upon graduation, although TVUS education in residency is not standardized. The objective of this study is to assess for improvement in TVUS knowledge among residents after viewing an educational DVD.

Methods: This is a multisite prospective randomized controlled trial using an educational DVD ("Gynecology: Beginners Only"), compared to routine education.

Are SMC Complexes Loop Extruding Factors? Linking Theory With Fact.

The extreme length of chromosomal DNA requires organizing mechanisms to both promote functional genetic interactions and ensure faithful chromosome segregation when cells divide. Microscopy and genome-wide contact frequency analyses indicate that intra-chromosomal looping of DNA is a primary pathway of chromosomal organization during all stages of the cell cycle. DNA loop extrusion has emerged as a unifying model for how chromosome loops are formed in cis in different genomic contexts and cell cycle stages.
