Medium-Term Forecast of Government Spending on the Unemployment Social Protection System in Russia in the Conditions of Economic Recession.

A simulation model of the Russian unemployment social protection system is developed and used to study the impact of the economic crisis caused by the COVID-19 pandemic, demographic processes, and the increase in retirement age on federal spending on unemployment benefits. Trends of overall and registered unemployment across various socio-demographic groups in 1992-2020 are analyzed, with special attention paid to periods of economic recession and recovery. The identified patterns are used to develop modeling scenarios.

[Diagnosis and treatment of upper urinary tract infection in women as a postoperative complication for treating genital endometriosis].

156 women operated on for genital endometriosis received treatment for infection of the upper urinary tracts diagnosed by ultrasound. The treatment was conducted only in case of normal urodynamics achieved by drainage (on demand) and comprised antibacterial therapy, efferent hemocorrection, intravascular laser irradiation of blood, hyperbaric oxigenation. A response was obtained in 98% patients.

Proportion of the GSTM1 0/0 genotype in some Slavic populations and its correlation with cystic fibrosis and some multifactorial diseases.

A homozygous gene deletion at the glutathione S-transferase M1 (GSTM1) locus of genomic DNA from blood spots was studied by PCR in the group of Slavic populations from the north-western and central-eastern regions of European Russia and in patients with lung cancer (LC), other tumors (OT), endometriosis (E), alcoholic cirrhosis (AC), cystic fibrosis (CF) and chronic bronchitis (CB). The frequencies of the GSTM1 0/0 genotype were 38.8% and 67.

[Pancreatitis associated with diseases of the stomach, duodenum and biliary tract].

The authors studied experience with simultaneous operations in 39 patients with coexisting diseases of the stomach, duodenum, biliary tract, and pancreas. A total of 31 operations were carried out in 14 patients with various forms of acute pancreatitis and 54 in 25 patients with chronic pancreatitis. One patient died in the immediate postoperative period from destructive postoperative pancreatitis.

[Draining surgery of the main biliary tract in the surgical treatment of chronic and acute pancreatitis].

The authors studied the site of draining operations on the biliary tract in 160 patients with biliary pancreatitis. Among 64 patients with chronic pancreatitis 41 (64.1%) were subjected to various draining operations on the biliary system, one patient died.

[Treatment of patients with genital endometriosis and nephroptosis].

The authors analyze the results of multiple-modality treatment of 31 patients suffering from genital endometriosis combined with nephroptosis. Nephropexy with a muscular flap was carried out in all the cases, followed by therapy for genital endometriosis, including hormones, immunomodulators, performed physical factors, hyperbaric oxygenation, etc. Late results were followed up for up to 3 years.

[X-ray diagnosis of urinary tract diseases in genital endometriosis].

The paper is concerned with the results of combined investigation of 157 women suffering from genital endometriosis, operated on for this disease. The involvement of the lower parts of the ureters in a pathological process was detected in 48 patients before operation and in 6 patients at operation. Comparison of x-ray and operative findings with histology specimens made it possible to define endogenous and exogenous forms of ureteral endometriosis and three variants of an x-ray picture in this pathology (the absence of abnormal changes of the urinary tract, hydroureter is over the narrowed part of the ureter, hydroureteronephrosis).

[The involvement of the urinary system organs in women suffering from genital endometriosis].

The data on the complicated character of endometriosis treatment, in females with the involved urinary system in particular, were discussed. The reported incidence of the affected urinary system in females with endometriosis ranged from 1-2 to 18.5 per cent.

[Terminolateral choledochoduodenal anastomosis].

A technique of the terminolateral choledochoduodenal anastomosis formation in 37 patients with impaired patency of bile ducts and indurative pancreatitis in presence of the common ampulla of the bile duct and pancreatic duct is presented. After the operation, the complications developed in 5 patients, 2 died. The result of treatment in observation of up to 4 years in 28 patients is good, in 2--satisfactory, in 2--bad (reoperated).

[Clinical course and surgical treatment of intestinal endometriosis].

The article presents clinical characteristics of the disease and an analysis of results of treatment of 33 patients with endometriosis of the colon. Ways of spreading endometriosis resulting in alteration of the intestine are shown such as the invasive, implantational and metastatic ways. Resection of the intestine wall with anastomosis in 3/4 after Melnikov was fulfilled in 15 patients with endometriosis of the sigmoid and superampullar portion of the large intestine.

[Indications for choledochotomy and methods for its performance].

Experience in the treatment of 238 patients for nontumorous lesions of the biliary tract by choledochotomy is generalized. Choledochotomy was performed in obstructive jaundice during the operation, in cholangiolithiasis, suppurative cholangitis, strictures of the terminal part of the choledochus, and indurative pancreatitis which were attended by biliary hypertension. Diagnostic choledochotomy was undertaken only in 5.

[Increased sensitivity to drugs and endometriosis].

Increasing incidence of predominantly polyvalent drug allergy was demonstrated in patients with endometriosis as an obstacle to their purposive treatment. For this reason, a system of measures aiming to prevent allergic and pseudoallergic reactions has been developed with respect to pathogenetic mechanisms involved, to be used in conjunction with conservative treatment, general anesthesia for surgical interventions and roentgenocontrast investigation in patients with endometriosis and polyvalent hypersensitivity to medication.
