Recalcitrant Pustular Dermatosis Successfully Treated With Prolonged Isotretinoin Therapy.

Isotretinoin (13-cis-retinoic acid) is a well-established systemic treatment for moderate to severe acne vulgaris, renowned for its ability to target multiple contributors to acne pathogenesis. However, its therapeutic potential extends beyond conventional acne management. This case report highlights its efficacy in treating recalcitrant pustular dermatosis, a condition that proved resistant to standard therapies and posed significant diagnostic challenges.

Development and testing of a remote hybrid SMS/web-based perioperative messenger: A mixed-methods study.

Background: Perioperative messengers as tools for hospitals to communicate with patients' loved ones during surgery have been described in form of pagers, SMS, email, and apps. The approach of combining multiple means of communication to create a secure and reliable messenger promises several advantages and has not been investigated to date.

Objective: The aim was to develop and test the technical feasibility and acceptance of a remote hybrid SMS/web-based perioperative messenger that keeps patients' loved ones informed around the time of surgery.

Herpes Zoster and Venous Thromboembolism Following Upadacitinib Treatment for Severe Atopic Dermatitis.

New medications targeting the Janus kinase (JAK)-signal transducer and activator of transcription (STAT) pathway have been developed through emerging biologics research. However, due to documented adverse effects, including herpes zoster (HZ) and venous thromboembolism (VTE), an extensive patient workup and counseling are necessary before prescribing. We present the case of an 81-year-old patient with severe atopic dermatitis on upadacitinib, a selective JAK1 inhibitor, who developed HZ and VTE, requiring hospitalization.

Development and external validation of temporal fusion transformer models for continuous intraoperative blood pressure forecasting.

Background: During surgery, intraoperative hypotension is associated with postoperative morbidity and should therefore be avoided. Predicting the occurrence of hypotension in advance may allow timely interventions to prevent hypotension. Previous prediction models mostly use high-resolution waveform data, which is often not available.

Complex Management of Basal Cell Carcinoma in a Frail Patient.

Basal cell carcinoma (BCC) is one of the most common cancers diagnosed in older patients and has low mortality. Surgical versus medical management is considered in patients with multiple comorbidities and limited life expectancy (LLE), where the risk-to-benefit ratio must be carefully assessed. Watchful waiting (WW) is a viable option for some patients with severe LLE when follow-up care can be provided vigilantly and frequently.

Art in nursing is present in various forms and there is ample literature exploring creativity, including journaling for clinicians, intensive care unit diaries for patients and providers, and music therapy for patients. Illness narratives, depictions of the sick, or the effects of disease are commonly represented in media. This article highlights how creativity and various mediums of artistic expressions may can be used as a self-care practice and aid in boosting empathy in health care providers.

Igniting Change: Supporting the Well-Being of Academicians Who Practice and Teach Critical Care.

Academicians who maintain a critical care clinical practice encounter numerous stressors, especially during the COVID-19 pandemic, which can influence well-being. This article provides historical perspectives on the stressors inherent in working in the critical care environment as well as the stressors of working in the academic environment. It proposes the application of the synergy model as a framework to help improve the well-being of academicians who practice and teach critical care.

Heroes against homophobia: does elevation uniquely block homophobia by inhibiting disgust?

Homophobia has decreased in past decades, but gut-level disgust towards gay men lingers. It has been suggested that disgust can be reduced by inducing its proposed opposite emotion, elevation. Research suggests elevation might reduce homophobia, but only general elevation (not elevation evoked by gay people) and general attitudes (rather than disgust) have been studied.

Innovative Classroom Upgrade: Simulating an Intensive Care Unit Environment in an Undergraduate Academic Classroom.

There are limited opportunities for undergraduate nursing students to take part in hands-on critical care. A course was developed to expose students to the critically ill patient in the intensive care unit environment and introduce skills that students may not have the opportunity to engage in during their clinical hours. Led by two faculty currently practicing in critical care, this course used multiple learning modalities, including high-fidelity simulation; didactic teaching; online discussion; and narrative, reflective writing, to teach critical care concepts and entry-level skills.

Negotiating Theory When Doing Practice: A Systematic Review of Qualitative Research on Interventions to Reduce Homophobia.

We performed a systematic review of qualitative research on interventions to reduce homophobia. Specifically, we conducted a thematic analysis of participant feedback given in 30 qualitative and mixed-methods studies. Participants often described interventions as "eye-opening.

A Cross-sectional Study on Evidence-Based Nursing Practice in the Contemporary Hospital Setting: Implications for Nurses in Professional Development.

Evidence indicates that nurses inconsistently engage in evidence-based practice (EBP). This cross-sectional study of 402 nurses at a medical-surgical hospital identifies strategies for augmenting EBP. Nurses' EBP beliefs scores were higher than their EBP implementation scores.

Review of maintenance of response to psoriasis treatments.

Background: Biologics are highly effective treatments for moderate-severe psoriasis with a high percentage of patients achieving 75% improvement in Psoriasis Area Severity Index (PASI75) at 12 weeks. However, psoriasis is a chronic disease, and long-term efficacy is critical.

Objective: Long-term responses of systemic psoriasis treatments and how this effect is reported.

Age-related changes in male forearm skin-to-fat tissue dielectric constant at 300 MHz.

Prior research suggests that tissue dielectric constant (TDC) values are useful to assess localized skin water in females for early diagnosing breast cancer treatment-related lymphoedema and TDC values in young adults have shown gender differences. However, no TDC data are available for older males nor have ageing effects been studied despite known shifts in water state and other skin age-related changes. Thus our goals were to (i) characterize TDC values at various skin depths in young and older males, (ii) determine the dependence of these values on body composition parameters and (iii) establish inter-arm TDC ratios for use as normal male reference values.

Interventions to reduce sexual prejudice: a study-space analysis and meta-analytic review.

Sexual prejudice is an important threat to the physical and mental well-being of lesbians, gay men, and bisexual people. Therefore, we reviewed the effectiveness of interventions designed to reduce such prejudice. A study-space analysis was performed on published and unpublished papers from all over the world to identify well-studied and underexplored issues.

Multiple nitric oxide sources in neurogenic plasma extravasation in rat hindpaw skin.

The contribution of nitric oxide (NO) to capsaicin-evoked plasma extravasation was studied in rat hindpaw skin. Two inhibitors of NO synthase were used: 7-nitroindazole, with a selectivity for nerve-derived NO, and the L-arginine derivative, N(omega)-nitro-L-arginine (L-NOARG), which is a non-selective inhibitor. Plasma extravasation was induced by intraplantar injection of 5 microg/50 microl capsaicin and measured by the Evans blue leakage technique.
