The centrifugal pump is the workhorse of many industrial and domestic applications, such as water supply, wastewater treatment and heating. While modern pumps are reliable, their unexpected failures may jeopardise safety or lead to significant financial losses. Consequently, there is a strong demand for early fault diagnosis, detection and predictive monitoring systems.

The elevated oxygen fraction and the increased pressure in the hyperbaric chamber dramatically increase the flammability potential of materials, leading to the need for rigorous standards to prevent flame ignition. A scientific method of evaluating the flammability risks associated with skin care products would be helpful.

The National Institute for Occupational Safety and Health (NIOSH) was requested to investigate three such incidents.
Methods: NIOSH conducted site investigations of the incidents, and the requesters also sent the involved oxygen resuscitators to a forensic engineering company for a causal analysis.
