Importance: Increasing evidence supports the oncologic safety of de-escalating axillary surgery for patients with breast cancer after neoadjuvant chemotherapy (NAC).

Objective: To evaluate the oncologic outcomes of de-escalating axillary surgery among patients with clinically node (cN)-positive breast cancer and patients whose disease became cN negative after NAC (ycN negative).

Design, Setting, And Participants: In the NEOSENTITURK MF-1803 prospective cohort registry trial, patients from 37 centers with cT1-4N1-3M0 disease treated with sentinel lymph node biopsy (SLNB) or targeted axillary dissection (TAD) alone or with ypN-negative or ypN-positive disease after NAC were recruited between February 15, 2019, and January 1, 2023, and evaluated.

Article Synopsis
 • The study investigates the safety of omitting axillary lymph node dissection (ALND) in patients with residual axillary disease after neoadjuvant chemotherapy (NAC), focusing on those treated with sentinel lymph node biopsy (SLNB) or targeted axillary dissection (TAD).
 • An analysis of two multicenter cohorts (MF18-02 and MF18-03) included 501 patients who received regional nodal irradiation and achieved a complete clinical response to NAC.
 • Results showed low recurrence rates (0.4% axillary, 0.8% locoregional) and no significant differences in disease-free or disease-specific survival between SLNB and TAD, suggesting ALND omission is safe if patients receive

 • This study compares two surgical methods, Hartmann's procedure (HP) and resection with primary anastomosis (RPA), for treating acute left-sided colonic emergencies among 1215 patients from 204 centers globally.
 • Results showed that while HP was the more common treatment (57.3%), RPA was favored for younger patients with fewer health issues and those needing surgery sooner.
 • The study concluded that although HP is still widely used, RPA might be the better option, emphasizing the importance of patient characteristics and surgeon experience in determining treatment choice.

Peptic ulcer perforation is one of the leading causes of acute abdomen, presenting with acute abdominal pain and severe distress for the patient. In one-third of patients, the presentation is less dramatic, resulting in significant delays in diagnosis. Herein, we present a very rarest case operated on for diffuse purulent peritonitis with double perforation of the stomach in a COVID-19-positive patient, which had a depressing outcome.

Article Synopsis
 • The ChoCO-W study aimed to examine the effects of COVID-19 on the clinical presentation and outcomes of acute cholecystitis, particularly focusing on the rise of gangrenous cases during the pandemic.
 • Over 2,800 patients from 42 countries were enrolled, with a notable 6.9% testing positive for COVID-19, revealing a significantly higher prevalence of preexisting conditions and more severe outcomes in this group compared to those without the virus.
 • Patients with COVID-19 experienced higher postoperative complications (32.2% vs. 11.7%), longer hospital stays (13.21 days vs. 6.51 days), increased mortality rates (13.4% vs. 1.7

Background: Many scoring systems have been developed for acute appendicitis, which is the most common emergent disorder in surgical practice. Considering the physiological changes and chronic diseases occurring with advancing age, an applied scoring system may not produce the same score in similar patients in all age groups.

Objectives: We aimed to compare the predictive values of scoring systems in different age groups.

Background: One of the most common peptic ulcer complications is perforation (PUP) which also remains an important cause of morbidity and mortality. In this study, it was aimed to compare the results of patients who had similar pre-operative scoring index results (Boey, Charlson Comorbidity Index (CCI) and Mannheim Peritonitis Index (MPI), and type of surgery.

Methods: Pre-operative Boey, CCI, and MPI scores were calculated by retrospectively examining the files of patients who were operated under emergency conditions with the diagnosis of PUP.

Meckel's diverticulum is generally asymptomatic, but it may become symptomatic due to various reasons and maybe the etiology of the acute abdominal syndrome. Bezoars are formed by the combination of non-digestible substances in the gastrointestinal tract, and which are among the rare causes of intestinal obstruction. The formation of bezoars in Meckel's diverticulum and subsequent intestinal obstruction is a rare condition.

Background: Traumatic rectal injuries are uncommon and can originate due to various causes. Rectal injuries have a high mor-bidity, regardless of cause, and detection at the time of occurrence is important to prevent fistula formation and/or stoma. In this article, treatment approaches in patients with isolated rectovaginal septum injury without perineal and sphincter injury during sponta-neous vaginal delivery are presented and the current literature is reviewed.

 • Breast cancer is rare in men and tends to increase with age, similar to women, but is less studied in male patients with neurofibromatosis type 1.
 • Neurofibromatosis type 1, an autosomal dominant genetic disorder, raises the risk of various cancers, yet breast cancer's characteristics differ between males and females, with males showing positive estrogen and progesterone receptors and negative HER2.
 • There is a need for better evaluation and follow-up recommendations for breast cancer in male neurofibromatosis patients, as current guidelines are lacking.

Objective: Cecal diverticulitis may be encountered as a real etiological factor in 1/300 appendectomies. Differential diagnosis of acute appendicitis and cecal diverticulitis is crucial because of the different treatment methods. Our aim is to reveal the importance of distinguishing acute appendicitis from cecal diverticulitis.

Introduction: Abdominal pain requires rapid diagnosis and treatment, especially in emergency circumstances. Sometimes the diagnosis of the disease cannot be accomplished with laboratory and imaging methods, and an invasive procedure such as diagnostic laparoscopy may be required to obtain a diagnosis. Diagnostic laparoscopy is also performed therapeutically, but laparotomy is inevitable in some cases.

Objective: In our study, we aim to evaluate in terms of patients the quality and reliability of the most relevant and most-watched videos uploaded on YouTube about pancreatic cancer.

Method: Before starting the study, YouTube search terms were determined by consensus by two General Surgeons. Then, on 10/10/2020, the terms such as "pancreatic cancer", "diagnosis of pancreatic cancer" and "treatment of pancreatic cancer" were entered separately in the search bar of YouTube, "relevance" was selected among the filtering options and the most viewed videos were listed.

Background: Although primer appendagitis epiploica (PAE) is an acute condition, there is no consensus about a standard first-choice treatment. Different non-surgical and surgical interventions for PAE are available.

Methods: In this study, a total of 39 patients who were diagnosed as PAE between 2013-2018 were evaluated retrospectively concerning recurrences of the disease, medical intervention, and the requirement of hospitalization.

Background: Although laparoscopic appendectomy increases its popularity today, the answer to the question of whether to perform open or laparoscopic appendectomy during pregnancy is appropriate in many studies, and the choice of surgery depends on the surgeon. Herein, we aimed to evaluate the variables that affect undesirable pregnancy outcomes that occur as a result of appendicitis during pregnancy.

Methods: Seventy-eight pregnant patients with acute appendicitis who underwent laparoscopic or open technique intervention enrolled in this retrospective study.

Background: Acute appendicitis is the most common non-gynecological emergency during pregnancy. The diagnosis of appendicitis during pregnancy is challenging due to changes in both physiological and laboratory variables. Guidelines suggest patients with suspected acute appendicitis should be stratified based on clinical scoring systems, to optimize the use of diagnostic imaging and prevent unnecessary surgery.
