Contemporary literature lacks examples of intradural, extramedullary spinal glomangiomas. Moreover, glomus tumors in general are exceedingly rare among benign spinal tumors and are mostly located within epidural space or within intervertebral foramen, and only a few cases have been documented to date. This report provides a detailed analysis of the clinical presentation, imaging characteristics, surgical intervention, and pathological findings of a 45-year-old patient experiencing progressive locomotor deterioration.

Markovnikov-selective double hydrosilylation of challenging terminal aryl alkynes under cobalt and iron catalysis.

Geminal bis(silanes) are unique compounds with interesting properties. The most straightforward way to access them is double hydrosilylation of alkynes, which was established only recently. Previous articles about transition metal-catalysed double hydrosilylation show that terminal aryl alkynes are a challenge.

Synthesis and Hemostatic Activity of New Amide Derivatives.

Eight dipeptides containing antifibrinolytic agents (tranexamic acid, aminocaproic acid, 4-(aminomethyl)benzoic acid, and glycine-natural amino acids) were synthesized in a three-step process with good or very good yields. DMT/NMM/TsO (4-(4,6-dimethoxy-1,3,5-triazin-2-yl)-4-methylmorpholinium toluene-4-sulfonate) was used as a coupling reagent. Hemolysis tests were used to study the effects of the dipeptides on blood components.

At Long Last: Olefin Metathesis Macrocyclization at High Concentration.

Macrocyclic lactones, ketones, and ethers can be obtained in the High-Concentration Ring-Closing Metathesis (HC-RCM) reaction in high yield and selectivity at concentrations 40 to 380 times higher than those typically used by organic chemists for similar macrocyclizations. The new method consists of using tailored ruthenium catalysts together with applying vacuum to distill off the macrocyclic product as it is formed by the metathetical backbiting of oligomers. Unlike classical RCM, no large quantities of organic solvents are used, but rather inexpensive nonvolatile diluents, such as natural or synthetic paraffin oils.

Facilitating the Timely Discharge of Well Newborns by Using Quality Improvement Methods.

Background And Objectives: Discharges are a key driver of hospital throughput. Our pediatric hospitalist team sought to improve newborn nursery throughput by increasing the percentage of newborns on our service with a discharge order by 11 am. We hypothesized that implementing a discharge checklist would result in earlier discharge times for newborns who met discharge criteria.

Axially chiral racemic half-sandwich nickel(ii) complexes by ring-closing metathesis.

A remarkable nickelacycle has been synthesised via olefin metathesis of the α,ω-diene complex [Ni(η-CHR)(Br)(NHC)] (R = C(CH)CHCH[double bond, length as m-dash]CH, NHC = 1-(6-hexenyl)-3-(2,4,6-trimethylphenyl)-imidazol-2-ylidene). Single-crystal X-ray analysis reveals a helical shape of the molecule and stretching of some interatomic distances in the Ni(ii) coordination sphere. The Cp-NHC tethered complex shows catalytic activity resembling that of the parent complexes.

Half-sandwich nickel complexes with ring-expanded NHC ligands - synthesis, structure and catalytic activity in Kumada-Tamao-Corriu coupling.

The general synthesis of [Ni(Cp)(X)(NHC)] complexes from a nickel halide, CpLi, and a carbene solution is reported. This procedure yields unprecedented complexes with ring-expanded NHC ligands (RE-NHC) of six- (1a, 1b), seven- (1c), and eight-membered (1d) heterocycles. The NMR spectra of 1a-1d are consistent with the hindered rotation of Ni-Ccarbene and N-CMes bonds, while X-ray analyses of 1b, 1c, and 1d reveal a pronounced trans influence of the RE-NHC ligands.

Objective: Food security is the ability to access sufficient food to maintain a healthy, active life. Previous studies link food insecurity with adverse health outcomes. Our objectives were to determine the prevalence of food insecurity, identify sociodemographic risk factors, and quantify receipt of public nutrition assistance among recently hospitalized children in the United States.

Perceived core competency achievements of fellowship and non-fellowship-trained early career pediatric hospitalists.

Background: The pediatric hospital medicine (PHM) core competencies were established in 2010 to identify the specific knowledge base and skill set needed to provide the highest quality of care for hospitalized children. The objectives of this study were to examine the perceived core competency achievements of fellowship-trained and non-fellowship-trained early career pediatric hospitalists and identify perceived gaps in our current training models.

Solid-state structures of two cationic complexes (2c, 2j) and two complexes with a coordinated anion (3a, 3c) have been determined by X-ray diffraction revealing a trigonal planar geometry in all cases. Synthesis and evaluation of bifunctional sGC regulators: optimization of a connecting linker.

Hybrid molecules composed of PpIX and cobyrinic acid derivatives conjugated through linkers of varying length and composition were prepared via 1,3-dipolar cycloaddition (CuAAC) or amidation/esteryfication reactions. They were tested for activation of soluble guanylyl cyclase (sGC), a key enzyme in the NO/cGMP signaling pathway, by an in vitro GTP→cGMP conversion assay. Using purified heme-deficient sGC and truncated sGC variants lacking a heme-binding domain, we demonstrated that such hybrid molecules may activate sGC by targeting heme-binding and/or catalytic domain.

Telepathology recently entered a new era with the introduction of digital microscopes combined with Internet technology. The microscope allows viewing real time of whole slide (macro) as well as different chosen fields in four different magnifications. Three Nikon Coolscope were installed in NHLS laboratories in Mthatha, East London and Port Elizabeth.

Besides, the few remaining specialist are often isolated from colleagues.

Diagnostic telepathology: long-term experience of a single institution.

Objectives: The paper reviews the development of the application of telepathology in a department of surgical pathology between 1991 and 2003. The goal of the efforts during this time was to give up the concept of programming a single application, available only between two fixed workstations with sophisticated devices and special software, and to find the virtual "largest common denominator" for implementing as many different applications as possible with the same basic system.

Methods: A new telepathology system was designed as a client-server system with a relational database at its centre.

Squamous cell carcinomas of the esophagus, a disease with poor prognosis, are especially frequent in China and South Africa. To initiate the study of endogenous lectins in this tumor class we employed synthetic neoglycoconjugates and focused on galectins as markers. Histological sections of 43 cases of esophageal carcinomas were analyzed with labeled galectins-1 and -3 and their specific antibodies, neoglycoconjugates exposing chemically prepared histo-blood group A-, B- and H-trisaccharides and the antibody MIB-1 (Ki-67).

Carcinoma of the uterine cervix: results of Ka-Ngwane screening programme and comparison between the results obtained from urban and other unscreened rural communities.

Objective: The Ka-Ngwane screening programme was initiated by the Department of Anatomical Pathology and the Department of Community Health at Medunsa to establish the incidence of cervical intraepithelial neoplasia (CIN) and cervical carcinoma (CaCx) in previously unscreened rural population in our catchment area.

Study Design: Ten thousand consecutive PAP smears from Ka-Ngwane (Mpumalanga) area are the subject of the analysis. The incidence of CIN I-CIN III and CaCx is calculated in the screened material.
