Tetralogy of Fallot with unilateral absence of pulmonary artery (PA) is a rare congenital anomaly that has been reported in isolated case reports and small case series. There is no well-defined treatment algorithm for these patients, and repair has been associated with high mortality, although survival is improving in the more recent era. Recent reports suggest strict case selection criteria based on PA dimensions and size of the left ventricle.

A 29-year-old female was diagnosed to have adult-type anomalous left coronary artery from pulmonary artery (ALCAPA). She underwent a two-coronary system repair using left internal mammary artery and right internal mammary artery Y (LIMA-RIMA Y) off-pump coronary artery bypass grafting and made an uneventful postoperative recovery. The usage of off-pump LIMA-RIMA Y graft can be a suitable option for adult ALCAPA with a potential for good long-term outcomes.

Objective: Cardiopulmonary bypass (CPB) initiated with high oxygen levels may expose cyanotic children to reoxygenation injury. The ideal method of initiation of bypass to prevent this phenomenon still remains largely unproven. This study tested the hypothesis that controlling oxygenation during initiation of CPB improves early postoperative outcomes.
