Purpose: Endovascular interventions have become the primary choice for treating complex aortic pathologies, particularly with the use of fenestrated and branched endografts. However, limitations, such as manufacturing time have restricted their applicability in urgent situations. This has led to explore alternative off-the-shelf solutions, including in situ fenestration.

Invasive aspergillosis is an extremely rare condition in healthy and immunocompetent individuals, and very few cases have been reported in previously healthy, pregnant, or postpartum women. We describe a case of invasive aspergillosis in a puerperal patient and present literature review results. We present a case of fulminant invasive pulmonary aspergillosis with cerebral, cardiac, and gastric involvement in a young woman, occurring a few days after an elective cesarean section.

Background: Bloodstain Pattern Analysis (BPA) is a forensic scientific discipline used to resolve criminal events. By studying the size, shape, and distribution of the bloodstains that constitute one or more bloodstain patterns, it is possible to determine the physical events responsible for their creation, as well as the positions and movements of the victim and, in cases of homicide, the perpetrator during the act.

Materials And Methods: BPA analysis was applied as a support in the reconstruction of the event dynamics in four forensic cases, in addition to the data collected during on-site inspections and instrumental investigations including PMCT, autopsies, histological, and toxicological analyses.

  • SUDEP is when someone with epilepsy suddenly dies without a clear reason, and tests show nothing caused it.
  • This study looked at two specific cases of SUDEP and reviewed past research to better understand it.
  • Researchers found that certain brain areas may cause problems with heart and breathing control during seizures, and they stress the need for thorough investigations for people who die from this condition.

Background: Cadaveric identification represents a legal, moral and medico-legal obligation and also fulfills the ethical function to allow the living to offer their dead a merciful burial. In the Mediterranean basin we have been witnessing for years a struggle for life where hundreds of migrants die in an attempt to reach Europe. For these people right to be recognized is not easy because of ante-death biological data are not always available for comparison, and DNA analysis is not always feasible cause for the absence of data with which to compare.

Background: Tracheal injury may be a rare complication of the endotracheal intubation procedure. Incidence and determinant factors are not well known, nevertheless a greater incidence have been recognized with a difficult maneuver or the use of nitrogen peroxide. The therapeutic approach can be conservative or surgical, depending on the characteristics of the lesion and of the patient and therefore the outcomes of medico-legal interest can be different.

Basal cell carcinoma (BCC) is the most frequent malignancy in the general population. To date, dermoscopy is considered a key tool for the diagnosis of BCC; nevertheless, line-field confocal optical coherence tomography (LC-OCT), a new non-invasive optical technique, has become increasingly important in clinical practice, allowing for in vivo imaging at cellular resolution. The present study aimed to investigate the possible correlation between the dermoscopic features of BCC and their LC-OCT counterparts.

Sigma-1 receptor (σ-1R) modulates cellular signaling pathways, probably acting as a ligand operated chaperone. When activated, the receptor translocates from the interface mitochondrion associated membrane of the endoplasmic reticulum to the cell membrane. σ-1R was demonstrated in some brain regions, including the pineal gland, and was proposed to be involved in several cerebral processes, including neuroprotective responses against homeostasis alterations.

  • Cryptococcosis is an infection primarily affecting those with weakened immune systems, impacting the lungs and central nervous system, though it can also appear in other areas like the skin and eyes.
  • Primary cutaneous cryptococcosis (PCC) can occur in healthy individuals through skin injuries, but is rare and hard to diagnose due to its non-specific symptoms.
  • In a case involving an immunocompetent man, PCC was confirmed through lab tests and treated effectively with three months of oral fluconazole, leading to complete recovery without recurrence.

We report a case of a nodular granulomatous secondary syphilis histologically resembling tuberculids in a patient with positive quantiferon test and serology for syphilis. Polymerase chain reaction (PCR) analysis led to the diagnosis. We underline the usefulness of PCR in clinically and histologically doubtful cases in order to avoid misdiagnosis and delay treatment.

Background: Several diagnostic and therapeutic processes are regulated by guidelines and recommendations provided by International and National Societies which offer standardized indications for health-care practitioners, including the treatment of pathologies that may affect the thyroid gland. These documents are very important for patient health promotion and, also, for preventing adverse events associated with patient injuries and related malpractice litigations. Particularly, thyroid surgery can be a cause of professional liability due to complications from surgical errors.

Background: Postmortem assessment of anaphylactic death is a challenge for forensic pathologists. One of the most frequent elicitors of anaphylaxis is insect venom. Here, a case of anaphylactic death due to Hymenoptera stings is reported to highlight the contribution of postmortem biochemistry and immunohistochemistry in assessing the cause of death.

Background: In recent years, due to the increase of complaints for medical malpractice, the Sicilian Regional Health System has adopted proceedings for the direct management of claims by each healthcare facility with the aim of reducing costs of insurance premiums and their relative taxes. Thus this management has led to increased awareness and management of clinical risk through the introduction of mandatory sentinel event monitoring.

Case Report: A 55-year-old man with acute ischemic heart disease, due to three-vasal coronary diasease, underwent angioplasty perfomed on the second diagonal branch of the anterior descending artery.
