Objectives: Current evidence supports the growing application of extended high-frequency (EHF: 9 to 20 kHz) audiometry in hearing research, which likely results from the high vulnerability of this frequency region to damage induced by known auditory risk factors. The present systematic review and meta-analysis were performed to investigate whether adults with a normal audiogram and tinnitus show increased EHF hearing thresholds relative to control peers.

Design: A comprehensive search was undertaken on electronic databases consisting of PubMed, ScienceDirect, Wiley, and Google Scholar using combined keywords: "tinnitus," "extended high frequency," "normal audiogram," and "hidden hearing loss.

Article Synopsis
  • Misophonia is a disorder where individuals have a low tolerance for specific sounds or their related stimuli, but lack of a clear definition has slowed research and treatment development.
  • From June 2020 to January 2021, a committee of experts used a modified Delphi method to create a consensus definition of misophonia through multiple rounds of voting, revision, and exclusion of candidate statements from existing literature.
  • The resulting definition, approved by at least 80% of the committee members, aims to unify researchers and clinicians in their efforts to understand and assist those affected by misophonia.

Cisplatin-induced ototoxicity in humans is more predominant in the cochlea than in the vestibule. Neither definite nor substantial vestibular dysfunction after cisplatin treatment has been consistently reported in the current literature. Inner ear hair cells seem to have intrinsic characteristics that make them susceptible to direct exposure to cisplatin.

Purpose: Meaningfully grouping individuals with tinnitus who share a common characteristics (ie, subgrouping, phenotyping) may help tailor interventions to certain tinnitus subgroups and hence reduce outcome variability. The purpose of this study was to test if the presence of tinnitus subgroups are discernible based on hearing-related comorbidities, and to identify predictors of tinnitus severity for each subgroup identified.

Methods: An exploratory cross-sectional study was used.

[Hyperacusis: a mini review].

Lin Chuang Er Bi Yan Hou Tou Jing Wai Ke Za Zhi

June 2021

Hyperacusis refers to the experience of the sound environment being perceived as overwhelmingly loud or painful, and this can severely affect the patient's quality of life. In China little attention has been paid to hyperacusis, and there has been little research undertaken on the topic. Therefore, this article summarizes the recent research on hyperacusis regarding definition, epidemiology, etiology, mechanism, evaluation and treatment to give directions for future research.

Article Synopsis
  • A study investigated the prevalence of hyperacusis (sensitivity to sounds) and its connections to hearing, mental, and general health among 5,107 people aged 45 to 70 in a community setting.
  • The findings revealed that 15.2% of participants reported hyperacusis, with many noting it occasionally affected their daily lives; factors like being female, older, having lower income, and existing health issues were linked to higher rates of hyperacusis.
  • The research highlights the importance of understanding the relationship between hyperacusis and overall health, suggesting that those with hearing loss or mental health challenges could experience more significant impacts from this condition.

Article Synopsis
  • Tinnitus affects the health-related quality of life (HRQoL) of patients and requires various treatment approaches, but the economic assessments of its impact (using QALYs and WTP) have not been widely explored in research.
  • A systematic scoping review identified 15 studies related to tinnitus that evaluated QALYs and WTP, revealing a scarcity of research utilizing direct preference-based measures (PBMs).
  • The review concludes that more robust studies using economic assessment methods are necessary to enhance healthcare decision-making and improve patient-centered care in the treatment of tinnitus.

Article Synopsis
  • Tinnitus experiences vary significantly among individuals, necessitating better understanding and management strategies based on distinct subgroups of severity.
  • A study of 326 adults identified insomnia, hearing distress, and anxiety as the main predictors of tinnitus severity, highlighting the importance of comorbidities over demographic factors.
  • The findings suggest that individuals with more severe tinnitus require urgent care, and treatment should prioritize addressing related conditions such as anxiety and hearing disability.

The identification of phenotypes within populations with troublesome tinnitus is an important step towards individualizing tinnitus treatments to achieve optimal outcomes. However, previous application of clustering algorithms has called into question the existence of distinct tinnitus-related phenotypes. In this study, we attempted to characterize patients' symptom-based phenotypes as subpopulations in a Gaussian mixture model (GMM), and subsequently performed a comparison with tinnitus reporting.

As for hypertension, chronic pain, epilepsy and other disorders with particular symptoms, a commonly accepted and unambiguous definition provides a common ground for researchers and clinicians to study and treat the problem. The WHO's ICD11 definition only mentions tinnitus as a nonspecific symptom of a hearing disorder, but not as a clinical entity in its own right, and the American Psychiatric Association's DSM-V doesn't mention tinnitus at all. Here we propose that the tinnitus without and with associated suffering should be differentiated by distinct terms: "Tinnitus" for the former and "Tinnitus Disorder" for the latter.
