A dynamic examination of 135 patients with transient disorders of cerebral circulation (TDCC) was conducted in conditions of a sanatorium-resort treatment in Kislovodsk in early stages following the last attack (from 2-3 months). The method of a phasic examination and treatment was accomplished according to the following principle: polyclinic -- hospital -- resort -- polyclinic. Apart from the clinical dynamic examinations such methods as EEG, REG studies of blood coagulation and basic metabolism, and if necessary angio- and dopplerosonography were used.

Using a neurological, psychiatric, experimental neuropsychological and REG study with the use of functional tests, the authors studied 66 patients with different stages of dyscirculatory encephalopathy and 20 normals of the same age (a control group). These patients demonstrated different degrees of disturbed intellectual and mnestic activities, a drop of attention and general activity, the speed of reactions, as well as different forms of expressed psychoorganic symptoms developing on the background of diffuse and eventually of microfocal neurological symptomatology. The REG data revealed relation between the stage of the disease on the one hand and the state of the vascular tone, elasticity of the vascular wall, level of arterial pessure and pulse blood repletion, the state of the venous outflow on the other.

The authors studied 41 patients with acute disorders of cerebral circulation, a focus in the right hemisphere and 14 with foci in the left hemisphere. The traits of compensatory reactions were studied according to the background and functional EEG, depending upon the severity, stage, localization and lateralization of focal vascular brain lesions. The authors indicate to a rigidity of clinical and EEG changes in patients with right hemispheric localization of the focus which is explained by the authors by disturbances of the higher cortical functions in these patients.

The paper deals with a study of 30 patients with transient disorders of circulation in the vertebro-basillar basin. Besides the routine methods of clinico-laboratorial examinations, for the first time in neurological practice a new method of electrophysiological registration was used: superslow fluctuations of the brain biopotentials. The patients were examined during the interattack period.

[Functional asymmetry of the human cerebral hemispheres].

Zh Nevropatol Psikhiatr Im S S Korsakova

April 1976

On the basis of a critical analysis of Soviet and foreign literature, as well as on some results of long-term personal observations, the authors discuss some modern concepts (clinico-diagnostical, neurosurgical, rehabilitative, neurophysiological) of functional asymmetry in the human hemishperes. The authors underline a necessity for a detailed multidisciplinary study of this problem, having in view its importance for the theory and practice of medicine.
