Article Synopsis
  • Hyperammonemia caused by carbonic anhydrase VA deficiency is a rare but severe genetic disorder linked to mutations in the CA5A gene, often resulting in unexplained hyperammonemia in newborns and infants.
  • A case study details a 5-year-old patient with a specific mutation in the CA5A gene, previously identified in a Russian boy, highlighting its occurrence in that population.
  • The study suggests that targeted genetic testing for this mutation should be implemented in neonatal intensive care units to allow for early diagnosis and intervention.

Background: Non-alcoholic steatohepatitis (NASH) is an inflammatory subtype of non-alcoholic fatty liver disease (NAFLD) and recent non-invasive test, MACK-3, demonstrated good diagnostic characteristics for fibrosis in several studies. The aim of our study was to assess the liver state using the non-invasive MACK-3 Test, and its link with liver fibrosis stage, inflammatory activity, steatosis, and metabolic markers in NAFLD patients with hypertension (HT).

Methods: Thirty adult NAFLD patients with HT, aged 30 to 60 years (mean age 46.

Background: The initial stage of the biosynthesis of steroid hormones in animals occurs in the mitochondria of steroidogenic tissues, where cytochrome P450 (CYP11A1) encoded by the CYP11A1 gene catalyzes the conversion of cholesterol into pregnenolone - the general precursor of all the steroid hormones, starting with progesterone. This stage is missing in plants where mitochondrial cytochromes P450 (the mito CYP clan) have not been found. Generating transgenic plants with a mitochondrial type P450 from animals would offer an interesting option to verify whether plant mitochondria could serve as another site of P450 monooxygenase reaction for the steroid hormones biosynthesis.

Objective: Despite the importance of blood flow regulation (BFR) in cerebral vessels, in particular, during organism development, there is no studies in children in the first weeks of life. We studied biochemical autoregulation of blood flow in infants born at 24-25 weeks of gestation.

Material And Methods: Eighteen infants with body mass 763±138 g were included in the study.

Submitted in the paper are basic mechanisms of development of disorders of gastrointestinal motility and evacuatory function in ulcer of the stomach and duodenum. Pharmacological correction of the above disorders is considered in some detail and discussed as thoroughly as possible. Ways for enhancement of motility and normalization of the evacuatory function of the stomach and duodenum are outlined, indications and contraindications are determined, specificities of each of the drugs employed are described.

Overall thirty-six patients with gastric and duodenal ulcer were evaluated for the clinical effectiveness of ethonium. The results obtained showed sufficient effectiveness of this drug preparation in the therapy of the above medical conditions. The mild course of ulcer disease appears to be treatable by monotherapy, while moderately severe and severe courses require incorporation of other medicinal agents into therapeutic plans, specifically, those from the group of blockers H2-histaminic receptors.

With the purpose of increasing effectiveness of therapy and preventive treatment of those patients having been in the zone of the Chernobyl accident, microwave resonance therapy (MRT) was tried in a multiple-modality treatment of this patient population. There were 36 male patients with a mean age of 37.7 years.

The studies covered 35 subjects with chronic cholecystitis who had to work in radionuclide-contaminated territory. There appeared changes in examinees' lipid peroxidation and antioxidant defense systems. These were more pronounced in leukocytes, possibly due to direct action of radiation on cell membrane, and may contribute to disease chronicity and recurrences.

Reactivity of organism was studied in a total of 48 patients with chronic duodenitis, actually in the zone of increased radiation. Hypo- and normotensive type reactions were found to prevail, each having its peculiar clinical picture. While studying the lipid peroxidation- and antioxidant defence systems, changes were identified in those parameters characterizing the above systems, their degree being higher in blood leukocytes.

These patients require individual rehabilitation after determination of the unspecific reactivity of the body with prodigiosane. Depending on the results of this test the patients were treated with drugs stimulating the immune response in case of hyporeactivity, with adaptogens in normoreactive course of the disease and drugs modulating B-cell immunity in hyperreactivity. This approach allows to reduce hospital time, enhances remission.

A study is presented of 107 patients with chronic hepatitis and liver cirrhosis verified on the basis of morphological changes of the punctate obtained at echocontrol puncture liver biopsy. It was established that clinical manifestations of chronic hepatitis and liver cirrhosis are determined not only by their morphological forms, but also by the structure of the inflammatory infiltrate of the liver tissue. This is to be considered in evaluation of the severity of the disease course and differentiated treatment.

Serum levels of eicosanoids in patients suffering from chronic hepatic diseases were analysed in correlation with morphological characteristics of the hepatic lesion, body reactivity, manifestations of the alternative and immune phases of the inflammation. The results could be contributing to diagnosing and treating chronic inflammation in the liver.

The paper provides the results of studying the clinical disease manifestations, the plasma content of fibronectin and fibrin and fibrinogen degradation products in patients with chronic hepatitis (CH). It is shown that they are determined not only by the morphological form of CH but also by the structure of inflammatory infiltrates in liver tissue.

A study is presented of relations in the clinical manifestations of the disease with age using methods of multifactorial statistical analysis in 203 patients with chronic hepatitis. The diagnosis of the disease was confirmed morphologically. Results indicate that the age factor produces a modifying influence on the clinical manifestations of the disease.

In young patients with borderline arterial hypertension the system of pressor and depressor prostaglandins (PG) is activated with maintenance of their physiological proportion. In patients with I stage essential hypertension a considerable rise in the level of PGF2 alpha and thromboxane determines the system imbalance in the direction of the predominance of pressor components. The platelet hemostatic link in I stage essential hypertension is characterized by an increase in platelet aggregation activity which is mediated by the imbalance of the prostacyclin-thromboxane system in the direction to thromboxane.

The method of echo-controlled percutaneous puncture biopsy of the liver is described. Use of the method extends indications to diagnostic biopsy of the hepatic tissue, allows to realize aimed removal of material from the given portion of the organ and increases safety of the procedure. Therapists, gastroenterologists and other medical specialists are advised to use this method.
